ON THE COVER

A TRAUMA
SURGEON
TO THE CORE

DR. RED DUKE LIVES BY HIS

WORD; SAYS LIFES MORE LIKE
CHESS NOT CHECKERS

At 79, Dr. James Red Duke whose face is just as iconic

as the sound of his voice could easily get a pass from
24-hour on-call duty or the grungier tasks of being an
emergency room trauma surgeon at Memorial Hermann
Hospital. But Duke, the father of the Life Flight helicopter
program, would have none of that. He still works too
many hours to count, reports in for marathon hours
and teaches the finer points of medicine to interns and
residents. He took time from his busy schedule recently to
talk to Chronicle Health editor Diane Cowen about good
health and his lengthy career in medicine.

Q: You spent years on TV talking about good health

and nutrition, too. Do you walk the walk?
A: I try to eat right and have for years. I really did learn a
lot by doing those TV programs. I wrote all that myself;
Ive always been kind of a health nut.
Q: So what do you eat?
A: First, I graze. I dont eat big meals. Today for breakfast
I ate grapefruit, cantaloupe, strawberries and blueberries
and some oatmeal. Midmorning I had some cottage
cheese. Then I had red beans and cornbread for lunch
and I just ate some veggies from a tray in the doctors
lounge. Tonight Im eating dinner with my godson and
another doctor whos a good cook.

Q: Do you take any vitamins or supplements?

A: I take all kinds of things. I take vitamins C, D, E
every now and then. Fish oil. Niacin, Folate, B12, Zinc,
magnesium, calcium. Im probably leaving something out.

Q: Anything you must have every day?

ROBERT SEALE
FOR THE CHRONICLE

A: A glass or two of red wine every night. Red grapes

are loaded with flavonoids and good antioxidants.
Drinking a little red wine every day can lower the
incidence of darned near anything. I think thats in the
Scriptures, too.
Q: ER doctors seem to work incredibly long hours.
Whats your workday or workweek like?
A: I dont even know how many hours I work. At 5:15
a.m. I take my dog to day care and I stay at work however
long it takes.
Q: What kind of exercise do you do?
A: I like to work out at the UT Rec Center. I warm up on the
bicycle and like the rowing machine. I go through a series
of weight training. I dont try to prove that Im 18 anymore.
When Im really working out, Ill do it four to five times a

week. Lately I havent been doing
anything but climbing stairs here
at the hospital. That will burn
more calories than anything.
Q: A doctors job let alone
that of a trauma surgeon is
stressful. How do you deal
with stress?
A: Thats a funny thing. It has to
do with the way youre wired, I
suppose. I dont get excited. The
crazier it is, the better I like it.
You could say Im an adrenaline
addict and thats probably true.
Q: Over the years you talked
about many health topics.
Some were fads, but many
were worth the time. What
health trends were worth it?
A: Well, I think people got the
message about sunscreen. A
certain segment of the population
became aware of their diet and
the quality of what they were
eating. And look at the people
who exercise now. I jogged back
in the 60s when nobody did
it. We might have made some
headway in safety wearing seat
belts makes a lot of difference.
Bicycle helmets, too.
Q: Do you have any health
concerns?
A: Mainly that Im getting old.
I think I can see better than
the residents. I am pretty deaf
and thats because I was a tank
officer in the Army and those
tanks are hard on your ears. One
ear isnt any good at all and the
other is fair.
Q: The last word?
A: I always come back to safety.
If people are conscious and
aware, like at stoplights, theres no
reason to hurry to get through a
stoplight. We see so many people
hit in intersections. Be aware
of your environment and what
youre doing. Think ahead, and
youll avoid all that stuff. Think,
damn it, think. Ill bet I say that
100 times a day. Were not playing
checkers, were playing chess.

TIPS

Become knowledgeable
about and committed to eating
the right foods. We see so much
obesity; its a problem in so
many ways.
Keep your brain busy,
especially if youre older. You
want to be interested, involved
and stimulated.
Its stupid to smoke. If you do
it, quit.
Agree to someday be an organ
donor by signing an advanced
directive and make sure your
family is aware of it. (Signing
the back of your drivers license
doesnt necessarily get it done).

PRODUCT TEST
WATERPROOF BANDAGES

Whether youre hitting the beach or pool, washing your car or working
in your garden, nows the time for bandages to do some heavy lifting.
We tested the waterproof variety, all of which claimed to stay on until
you want them off and to seal out dirt and contaminants. We gave
all products two tests: a sweaty lawn-mowing session as well as an
afternoon spent wade fishing. Heres how they fared.
DIANE COWEN

BY KIM MORGAN
FOR THE CHRONICLE

Theres really no better

way to beat the heat of a
south Texas summer than
hitting the pool, lake or beach.
Without proper supervision or
swimming skills, a fun day of
play can turn to tragedy.
According to a Centers for
Disease Control report, there
were 312 drowning deaths in
Texas in 2005.
Children ages 0-4 accounted
for 61 of those; 49 were
children ages 5-19; and 58 were
adults ages 20-29.
Drowning is the second
leading cause of unintentional
death among children ages 114, said Teena Sandberg, Red
Cross water safety instructor
and water instructor for South
Montgomery County YMCA.
Half of all childhood accidents
are water related. Last year, 49
children died in the Houston
area. It takes less than 2
minutes for a child to lose
consciousness. Within four to
six minutes, its brain damage.
Or death. And it takes less
than 10 seconds for tragedy to
occur.
So what do you do if you
notice somebody flailing in the
water?
Sandberg said theres a
saying they teach children, and
it applies to adults as well. Its
Reach, Throw, Dont Go.
Reach means keep
yourself out of harms way
while helping someone. You
can get on your stomach and
reach out with a piece of
equipment or a towel.
Throw means toss an
item to the swimmer, such as
a noodle, an empty ice chest, a
seat cushion, anything they can
hang on to that wont weigh
them down.
Dont go refers to resisting
the urge to jump in and grab on
to somebody who is panicking
and will likely pull you down,
too. So dont go in the water,
but do call 911.
The rules change, though, if
the swimmer is already at the
bottom of the pool.
Chris Crease, managing
director of health and safety
at the American Red Cross
Greater Houston Chapter, said
first make sure the area is safe,
because you dont know why

CHIR0PRACTIC CARE

FOR THE CHRONICLE

Youve seen them at the

pharmacy, and at one point or
another been curious enough
to try it.
Blood pressure monitoring
is not just for the doctors office
anymore.
People can buy monitors for
at-home use as well, a move the
American Heart Association
recently endorsed for the 72
million Americans who have
high blood pressure.
Dr. Randall Wright, medical
director of the Conroe Regional
Stroke Center and presidentelect of the Montgomery
County Board of Directors
for the American Heart
Association, said the message is
one of preventive care.
The at-home devices range
in price from $40 to $100.
Wright said hed like to
see the monitors covered by
insurance because of their
potential for better managing
high blood pressure and for
helping prevent secondary
diseases.
But do they work? And how
do you know which type to
get? How often should you be
monitoring?
Dr. Jon Heine, cardiologist
at the Memorial Hermann
Heart & Vascular Institute in
Memorial City, said they do
work, perhaps best when it
comes to catching patterns.
The easiest ones are the
digital ones, and I recommend
the arm cuff devices, because
they are more accurate than
wrist cuffs, Heine said.

The best buy

Consumer Reports, a
monthly magazine focusing on
product testing and published
without advertising, said
in an October 2007 report
that automatic arm monitors
provided the most consistent
readings.
The monitors work by
wrapping the cuff around
your upper arm and pressing a
button that causes it to inflate,
automatically reading your
blood pressure.
Wrist monitors
automatically inflate and
provide a digital readout, but
like the name implies, you wrap
the cuff around your wrist.
Consumer Reports said that
while the wrist monitors were

less uncomfortable, they were
also less consistent than arm
cuff models.
Gayle Williams, deputy
editor of Consumer Reports,
said her magazine last rated athome blood pressure monitors
in 2003.

Calibrate often
If you purchase an at-home
monitor, take it to your doctor
to check its accuracy.
We compare what theyre
getting to what were getting,
Heine said. The readings are
surprisingly close.
Testing recommendations
Heine recommends
monitors for people being
treated for hypertension to
make sure their therapy or
medication is keeping their
blood pressure under control.
But he urges caution.
Some people literally get
addicted to taking their blood
pressure, Heine said. They
take it all the time and then
call the doctor the minute its
a little bit high. Blood pressure
by itself is not going to make
you have a stroke the hour your
pressure is up.
Some experts recommend
daily testing, but Heine said
that may be too frequent for
some people. He recommends
two to three times a week at
variable times.
And it never hurts to stick
your arm in the cuff next
time you see a machine at the
pharmacy, Heine said.
People go in there for
medication or something, and
while theyre waiting they
stick their arm in there and see
their blood pressure is a little
higher than it should be, so
they come in to be checked,
Heine said. Of course thats a
good thing.

Dr. Brandi Hill

CHIROPRACTOR

CHIROPRACTIC AND ASTHMA

What is Asthma?
Asthma is inflammation (swelling) of the airways that carry air in and
out of your lungs. The inflammation makes the airways very sensitive
and they react strongly to irritants or allergens. When they react, they
get narrower and less air flows through your lung tissues.
What are the Symptoms of Asthma?
Wheezing (a whistling sound when you breathe), coughing, chest
tightness, and trouble breathing are the most common symptoms.
When the symptoms become worse than usual, its called an attack.
Severe attacks can be life threatening because the airways close off so
much that not enough oxygen gets to vital organs. This condition is a
medical emergency.
What Causes Asthma?
It is not clear what causes the airways to be inflamed in the first place.
Many things can bring on an attack, including allergens (pet hair, dust,
pollen, mold), medications, cigarette smoke, exercise, and changes in
weather. If you have asthma, you should see your doctor regularly. You
will need to learn what things cause your asthma symptoms and how
to avoid them. Your doctor will also prescribe medications to keep your
asthma under control.
How Can Chiropractic Help?
Chiropractic is not a treatment for asthma. However, many patients
who suffer from asthma report improvement by receiving chiropractic
care. Your diaphragm is the primary muscle responsible for breathing.
Nerves that control the diaphragm exit the spinal cord in the mid-neck
at the C3, C4, and C5 area. Spinal misalignments in this area can
cause irritation of these nerves and interfere with proper function of
the diaphragm and lungs. Chiropractic care maintains proper alignment
of the spine thereby reducing irritation of the nerves and restoring
proper function to the diaphragm and lungs.
In fact, research published in the Journal of Vertebral Subluxation
Research documented the results experienced by 81 children with
asthma who received chiropractic care. The two-month study revealed
that those under care saw a 45% decrease in the number of attacks
and that 31% of the subjects voluntarily chose to decrease their
medication.
Naturally, we cant make any promises, and we never advocate
patients decreasing or stopping their medications without talking to
their medical doctor, but if you or your child suffers from asthma, call
Marshall Back and Body Wellness Center! We may be able to help!

PAM ANDERSON
COOKBOOK AUTHORS LATEST
HAS A PERSONAL SIDE

&

When you
write cookbooks and test recipes
professionally, its hard to find
or maintain your perfect
weight. All that baking, tasting,
re-baking and re-tasting can
add up to many extra pounds.
So when cooking expert and
award-winning cookbook author
Pam Andersons two teens flew
the coop, she took stock of her
life. The result was a woman
who learned how to let some
things go, resolved lingering
issues with her parents, gained
a new hobby (running) and lost
45 pounds. She recently visited
Houston and talked to Chronicle
Health editor Diane Cowen
about her new health cookbook,
The Perfect Recipe for Losing
Weight and Eating Great.
HOUGHTON MIFFLIN

STAYING FIT: Pam Anderson is the author of The Perfect Recipe for

Losing Weight and Eating Great. She lost 45 pounds three years ago and
gained a new hobby running.

Q:

Your book is full of

recipes, but its heart
is your personal story

family issues, weight issues,

therapy. Was it hard to make
yourself that vulnerable?
I had gotten used to
telling my story when
it was about a particular dish,
perfecting it, and my insecurities
as a cook. I felt like it was time to
open up. I hadnt heard
people talk about weight loss
from this perspective. I am
someone who lost a significant
amount of weight and kept it
off for three years, so I have
something to say.

A:

Q:
A:

You say the way you

eat now isnt a diet,
but its definitely a plan.
Right?
I eat three smaller meals
and two snacks a day,
about 2,000 calories. Every fit
person I know has boundaries
or a system. Most people want
other people to come up with it
for them. I hope people will read
my story and use my recipes to
come up with a way of eating that
works for them.

Q:
A:

Magenta

What have been

your biggest dietary
changes?
Portion size. I never
stopped eating pasta but I
did start weighing it. I measure
nuts; I dont just eat them from
the jar.

Is there any food you

simply stay away

from?
I dont keep junk food in
the house. A lot of people
have a sweet tooth. I have a
savory tooth. I find a handful of
nuts are rich, but are not empty
calories.

were already eating?
Theyre a combination of

recipes that I thought would be

Q:
A:

Theyre not necessarily

all low-fat or diet
foods. Are they prepared
differently?
I believe that food should
be satisfying and flavorful.
I spike my food with a lot of
flavor. Lots of herbs, spices,
zests. Flavorful, but judiciously
used, cheese like feta, parm,
goat. If you flavor just the surface
of things you dont need as much.
Instead of dunking a coconut

need.

Technology brings new options to treat allergies, asthma

INFORMATION

BY DALE ROBERTSON
HOUSTON CHRONICLE

Houston can be a hellish place

for allergy sufferers. Our heat,
humidity and notoriously dirty
air, smelling of chemicals, have
long created a stew of discomfort
for those afflicted with hay fever
and asthma.
But the hard evidence
presented by your runny nose,
stuffed-up sinuses, bloodshot
eyes and constricted chest
notwithstanding, were not even
close to being Americas allergy
capital. In the current rankings,
there are 38 cities across the
country more put-upon by
upper-respiratory afflictions and
71 have us beat on the asthma
front.
So, yes, things could be a lot
worse. And, truth to tell, things
actually appear to be getting
better, if only incrementally, as
our forward-leaning medical
community shifts its focus from
treating allergic responses to
attacking the root causes.
Further, the ban on smoking
in public places was a major
victory in the allergy wars,
allowing Houston to make a
quantum leap downward in the
standings over less enlightened

afflicted than ever before.

I think climate change has to
be considered as a major factor,
Chenault says.
Although Houstons allergies
are worse in the spring and the
fall, they rarely abate completely.
Global warming is, by all
accounts, keeping conditions
at their ripest for even longer
periods.
The problem with Houston
is that were very near to the sea
and we dont have much cold
weather, says allergist Dr. Juan
Zambrano. Once the pollen
comes, it doesnt necessarily
go away. The grass pollens are
particularly prolonged.
In addition to fostering the
growth of molds, the source of
some of the most serious asthmacausing allergies, our dank air
is also particularly hospitable
to dust mites, Zambrano said.
Almost nothing in Houstons
environment is a bigger asthmacausing problem.
And their far larger fellow
pests, cockroaches, are partners
in crime.
What to do? For starters,
cleanliness is next to godliness.
Frequent dusting and
removing knickknacks that
collect dust is recommended.
Further, no matter much you
may love wall-to-wall carpeting,
splurge on a hardwood or tile
floor on hypo-allergenic grounds.
Animal dandruff is frequently
fingered as a major allergy
culprit, although pet lovers can
take heart in knowing it may be a
less serious curse than was once
believed. Zambrano said research
shows that very young children
who are exposed to dogs and cats
actually fare better around them
as adults.
The normal reaction to stuffy
sinuses and/or lung congestion
is to mount a full-frontal attack

with pills and potions. But, while

the products that fill drugstore
shelves provide temporary relief,
they can have unhealthy side
effects. We tend to overmedicate
ourselves out of desperation,
perhaps benefiting only from a
placebo effect. And the constant
consumption of antibiotics, such
as Augmentim, have rendered
them increasingly ineffective in
combating sinusitis.
In the war on asthma, new
technology that measures the
lungs output of nitric oxide a
slam-dunk asthma indicator is
helping allergists more precisely
diagnose and treat the disease.
In turn, the most exciting
development is a procedure
called Balloon Sinuplasty.
Similar in concept to
angioplasties that treat
congestive heart failure,
the procedure has been in
widespread use for barely a year,
but doctors are excited about its
short-term results.
A soft, flexible guidewire is
inserted through the nostril to
the sinus cavity, then a balloon
catheter is advanced over it
and positioned across the
blocked sinus opening. When
the balloon is gently inflated,
it causes micro-fractures in the
bony edge of the sinus cavity
to eliminate the constriction,
restoring normal sinus drainage
and function. One Houston
practitioner, Dr. Cary Moorhead,
calls it a quantum leap forward
because its so minimally
invasive.
The less we disturb normal
tissues, Moorhead said, the
better the ultimate result because
we arent leaving behind any
scarring. Since there isnt lot of
cutting, the patients tolerate the
procedure better than traditional
sinus surgery. Theres a lot less
pain and discomfort.
In 75 percent of the cases in
which Moorhead has utilized the
Balloon Sinuplasty, no surgery
whatsoever was required.
The next step in the
procedures evolution is to
increase the precision to the
point that a general anesthetic
wont be needed. When thats
possible, Moorhead said,
Sinuplasty will move up as a
first-resort strategy, eliminating
the need for allergy shots and
medications for many.
Ideally, of course, ridding
ourselves of the trouble-causing
allergens in our environment
would be the best plan of action.
But we probably shouldnt
hold our breath waiting for
that to happen in Houston or
anyplace else.

One perfect tumble deserves another

BY LYNN COOK
HOUSTON CHRONICLE

There was a time lets call

it 1983 when I could do the
splits perfectly.
All my elementary school
pals could. We had entered
that beanpole stage and could
pretzel-twist ourselves into
practically any position.
Flash forward 25 years.
Flexibility is not my forte.
I probably should have
considered that before
signing up for a Tumbling and
Trampoline class for adults
at the Jewish Community
Center.
I can touch my toes, but if I
have to hold that position for
more than 30 seconds beads of
sweat break out on my forehead
and my hamstrings start to
scream. Thing is, I was seduced
by the trampoline part of the
course description.
Who doesnt love bouncing
on a trampoline? I hadnt been
on one in at least two decades,
but I was about to learn theres
so much more to T&T class than
jumping up and down.
The night I tried it out, I was
the only newbie in attendance.
One young woman had taken
gymnastics most of her life,
starting out in early childhood.
A man in the class had been
coming off and on for two
years. Its a great way to blow
off steam after work, he told
me.
These guys were good.
After a brief warm-up of
running short laps around
the gym, stretching and, yes,
attempting the splits, my
classmates were ready for
pikes, bounders and back
flips. Still staggered and
feeling the massive blood rush
from our warm-up headstand
something else I hadnt
attempted since third grade I
was more in the tuck-and-roll
stage.
Still, I never felt
uncomfortable. Coach Roumen
Yordanov gave me simple
moves to start out with as I
bounced down the long, low
trampoline runway. Jump,
jump, pike. Jump, jump, splits
in the air. Although the rest of
the class was doing back flips,
everybody cheered when I
managed to execute a wobbly
front handspring, head over
heels off the trampoline into a
plush, cushy mat.
Back in 1983 when I
was doing the splits on the
playground, Yordanov was a
gymnast training in Bulgaria.

ADULT TUMBLING

Clothes: T-shirts and
sweatpants.
Where: Jewish Community
Center, 5601 S. Braeswood
When: Wednesdays, 7:308:30 p.m.
Cost: $55 for four classes;

Today hes the head boys coach

for the Houston Gymnastics
Club at the JCCs main campus
in Southwest Houston. He
also teaches the T&T class for
grown-ups.

Yordanov attended a
special gymnastics school and
competed internationally,
including the 1980 Olympics in
Moscow. He again made
his countrys team for the 1984
Los Angeles games, but the
Soviet bloc boycotted the
event.
When the iron curtain finally
fell and once-flush socialist
gymnastics programs were left
without funding, Yordanov
emigrated to the U.S. Hes been
in Houston ever since.
In a year like this one when
the Olympic games feature
gymnasts, its common for kids
to want to try out their own

somersaulting, vaulting and

balancing abilities. Houston
Gymnastics Club has classes for
boys and girls of all ages and
levels of experience.
But adults get into
gymnastics, too. Thats why
Yordanov ends his once-a-week
adult tumbling and trampoline
class with 15 minutes of free
time.
Some went for coachsupervised time on other
equipment or freestyle floor
work. Some of us just wanted
one more go on the trampoline.
Jumping and attempting
handsprings and flips certainly
set my heart racing, but the

most strenuous part of the

class came at the end of the
night when I vaulted off a
trampoline into a giant pit filled
with foam squares to cushion
my fall.
It was a soft landing, but I
almost instantly felt like I was
in quicksand. Hauling myself
out of there was a workout in
itself. Aside from that indignity,
T&T was the most fun class Ive
ever taken.
Youre going to be sore
tomorrow. You need to go home
and take aspirin, Yordanov
told me. Major understatement.

The basics: The holy trinity consists of docosahexaenoic acid

(DHA) and eicosapentaenoic acid (EPA), both of which are found
in fish oils, and alpha-linolenic acid (ALA), from nuts such as
English walnuts, canola, soybean, flaxseed, linseed, etc. Fish
oil helps the blood circulate better and there is evidence that
it reduces blood triglyceride levels, lessening the risk of heart
attacks. Studies of DHA and EPA have shown they significantly
decrease the buildup of atherosclerotic plaques the socalled hardening of the arteries as well as lowering blood
pressure. ALA offers similar, if less scientifically established,
benefits. Research also suggests these fatty acids help fight
cancer and benefit children plagued with ADD and behavioral
problems.
A cautionary note: Some people have reported excessive
bleeding when consuming large amounts (over 3 grams per
day) while taking aspirin, and super-large doses are thought to
have on rare occasions caused hemorrhagic strokes. Another
reported negative side effect is immune system dysfunction,
and the omega-3s can be dangerous for people already in
advanced stages of congestive heart failure.
For expert advice: Numerous Web sites offer a variety of
educated opinions, most of them positive. If you already have
heart disease, your cardiologist should make the final call.
Our two-cents worth: For most of us, swallowing a couple
of the big gel capsules daily seems a good, reasonably cheap
heart-healthy insurance policy.

Gear Girl does love her shoes ... but she loves her gadgets, too. Here are a few to keep you entertained without emptying your wallet.

Marwares Sensor
Sportsuit ($10)
Gear Girl admits it. Shes
a sucker for tech gadgets,
especially those for her
Apple iPod or anything

bearable. GG was all gaga
with the Nike + Ipod Sport

putting the sensor chip

Then Marware, the
California-based tech
accessory maker, came up
with the Sensor Sportsuit.
Slip the chip into the suit
and Velcro that puppy onto
shoelaces. Works great.
Available at marware.com
and Amazon.com.

Nike SportBand ($59)

Merrells Ahnu ($80)

Nike has come up with an iPodless version of the same gadget. At

$59, some say this is an overpriced
pedometer. But GG likes her
gadgets simple. Not everything has
to be a junked up, overpriced cell
phone (Ah, if GG had a dollar every
time she heard a woman vexing over
her overloaded cycling Polar say: Why
cant I get this *@!% thing to work?).
The SportBand allows you to
read distance, pace and calories on
your wrist, instead of pulling your
iPod off your waistband every 10
minutes. GG would love to see a heart
monitor thats just as sleek and easy to
read. (Hint, hint, Nike.)

Hiking-shoe maker Merrell is finally beating the clunky

out of its river sandals. Even Nordstrom noticed and
featured both Merrells edgy-cool Waterpro Simar together
with Anhus Tilden in a recent catalog. GG tried a green pair
of the Tilden on a rafting trip recently. Loved them. Now
GG - a big sap for sexy gear that lasts - is saving up for the
sexier Waterpro Simar.
Available at REI, Whole Earth
Provision Co, Nordstrom and
Sun & Ski Sports.

Primus Atle BBQ Stove / Family Camping Stove ($90);

Keltys Basecamp Kitchen ($160)
Now for the non-athletic gear thats out there. Its
summer and more of us like to take the great indoors
with us on the camping trip. So if eating out of a propane
backpacker stove is not your idea of dining alfresco, then
try the Primus Atle BBQ Stove / Family Camping Stove.
It offers a propane-powered, non-stick grill surface with
traditional stovetop that folds flat. Pair it with Keltys
Basecamp Kitchen and bring out the outdoor gourmet in
you. Available at nagear.com, kelty.com

diabetes, which is characterized

by the bodys inability to
produce or properly use insulin,
a hormone that maintains
normal levels of blood sugar,
or glucose, and helps convert
sugar, starches and other food
into energy. About 5 million
inject insulin to treat their
diabetes, including as many as 3
million with Type 1.
In an effort to maintain
control of their glucose
- high levels damage blood
vessels and nerves and lead to
complications - conscientious
diabetics will prick their
fingers four to 10 times a day to
test their levels.
Studies have found that even
patients who measure their
glucose as much as nine times a
day spent less than 30 percent
of the day in the normal range.
In that context, the Food
and Drug Administrations
recent approval of continuous
monitors was a breakthrough.
The monitor, worn like a pager
and connected to a sensor
placed just under the skin, tells
the patient whether levels are
trending up or down.
The monitors made things
a lot easier for me, says Rob
Loar, a Kingwood native and
medical student diagnosed
with Type 1 diabetes in 1992,
when he was 6. Because of
the additional information it
provides Im able to use my

delivered at different times

and in different circumstances
need to be improved, though
researchers say that shouldnt
be an easy problem to solve.
Skeptics complain that they
have been hearing about an
artificial pancreas since the
mid-1970s, when prototypes
were refrigerator sized and
hospital based. More than 30
years later, hoping instead for a
biological cure, they argue its
still just a lot of hype.
More specifically, they
warn about the inevitability
of mechanical failure, which
could cause catastrophe; the
burden of carrying around even
more technology; and price tags
figure to be prohibitive.
Researchers respond that
they are working on back-up
systems in case of mechanical
failure, that later-generation
models of the artificial pancreas
likely would be wholly under
the skin and that insurers
will embrace it because it will
dramatically reduce the $100
billion annually on diabetesrelated medical care.
Its important to remember
that an artificial pancreas
wont be a cure, said Dr.
Philip Orlander, head of
endocrinology at the University
of Texas Medical School at
Houston. Itll still require
doctors visits and monitoring.
But it would be a major
improvement.

Whats even better than

an entire health center
devoted to diabetes care?
Nine of them.
If you have diabetes, your days of wasting time (and gas
money) driving all over town to find all the care you need
are finally over. Our nine Houston area offices all have
physicians who specialize in diabetes, on-site labs that
deliver your results during your visit, and Certified Diabetes
Educators who focus on your specific needs to personalize
your diet, exercise and lifestyle plans. One visit is all itll
take to see the difference. Plus we also accept most insurance,
and we offer all our same-day services for one easy copay.

But the monitors have
limitations. Their readings are
usually 15 to 30 minutes behind
real time. They require users to
be educated and committed to
the task, a requirement thats
resulted in short-term studies
showing mixed results. And
because adoption of the device
has been limited, insurance
only sporadically covers its cost
- $1,000 plus at least $350 a
month for supplies.
The fully realized artificial
pancreas should change all
that. Researchers predict it
would be universally adopted
by diabetics on insulin - and
thus covered by insurance because itd work automatically,
rendering patient involvement
unnecessary.
In the first pediatric trial
of a prototype of the artificial
pancreas earlier this year, Yale
University researchers tested
it on 18 teenagers who stayed
in a hospital for 36 hours and
found it effectively kept glucose
levels in the normal range
throughout. New trials testing
the system in less controlled
settings are in the works.
But researchers
acknowledge there is still
much work to be done. The
biggest challenge is developing
a sensor that delivers accurate
measurements of glucose
levels to the pump in real time.

Toss the razors and wax and go hairless

BY KIM MORGAN
FOR THE CHRONICLE

G A R Y F O U N TA I N :
FOR THE CHRONICLE

SAFE REMOVAL: Nancy Holmes

uses a laser to remove leg hair.

Sondra Rosenthal hasnt

shaved or waxed in years, but
its not because shes fond of
the au naturel look, especially
during summer.
Its just that now, instead
of tweezing, plucking, shaving
or waxing, she has opted
for permanent hair removal
using electrolysis and laser
treatments.
It took years to have
my underarms done by
electrolysis, said Rosenthal,
56. I had the laser on my legs
and one, two, three ... the hairs
were gone. There are still some
blonde hairs left, though.
Thats because the laser

used for hair removal isnt able

to target those lighter colored
strands, said Nancy Holmes,
owner of Hair Removal of
Houston and Hair Removal of
Katy.
While electrolysis has been
around for more than 100
years, she said, laser wasnt
approved by the U.S. Food and
Drug Administration until 1998
but they label it as permanent
hair reduction as opposed to
removal.
The laser does not recognize
non-pigmented or light colored
hair, so it reduces the amount
of hairs by only the dark
pigmented ones, Holmes said.
So far there is nothing in the
light-based laser world that will
do it. Electrolysis is still the only

way to remove blonde, white or

grey hairs.
Holmes said the laser is only
interested in the color of the
hair below the skin, so if youve
chemically treated your hair,
you might still be a candidate
for laser. But laser hair removal
works best on people with dark
hair and light skin.
For the rest of us, Holmes
recommends both laser and
electrolysis for the best results.
Is it safe?
According to Holmes,
youre more likely to get hurt
using your curling iron than
undergoing laser treatment. She
said that less than 1 percent of
people undergoing laser hair
removal suffer injuries.
Dr. Suneel Chilukuri, a

BIG LOSER.

surgeon at Memorial Hermann

Northwest Hospital, said
permanent hair removal
methods have improved
significantly since the
development of lasers in the
1980s.
But things can still go
wrong.
The follicle is heated to the
point of destruction, Chilukuri
said. Problems arise when
this heat is dissipated to the
surrounding skin. If the heat
dissipation is not properly
controlled, the skin becomes
either hyper-pigmented
(darker) or hypo-pigmented
(lighter). If the laser is used
at the improper energy level,
there can be permanent
discoloration.
Chilukuri said thermal
burns can also occur, looking
like a sunburn or oven burn and
can result in scarring.
Electrolysis comes with
similar risks, she said.
People should seek a skilled
provider who understands
the risks involved with hair
removal procedures and know
how to treat complications,
Chilukuri said.
If you do take that step to a
hairless face, neck, chest, back,
armpits, legs or bikini line, you
will want to know: Does it hurt?
There is a nominal amount
of pain involved in any hair
removal, but most people
tolerate it well, said Holmes.

cells at the base of the

Pros: Immediate results.
Cons: Requires treatments
until it becomes permanent
(usually 8-12 months).
Average cost: $30 for a 15
minute session.

LASERS
How it works: Light from

the laser passes through skin

and is absorbed by pigment
in the hair follicle, restricting
its ability to regenerate.
Pros: Works with less
treatments than electrolysis,
so its more cost effective.
Cons: Does not work on
blonde, grey or light-colored
hair
Average cost: Depends on
size of area treated. Female
client with hair on upper lip
will spend about $90 every
other month.

READ THE LABELS: Heidi Bragg, a pharmacist and University of

Houston professor, says certain foods and drugs shouldnt mix - such as
grapefruit and Lipitor.

DID YOU KNOW?

Thyroid medicines should be taken first thing in the morning

and without other medicines. It should never be taken with

iron, which will not allow the synthetic thyroid to be absorbed.
If a person battling cholesterol regularly drinks grapefruit
juice and does not want to alter his/her diet, the patient and
doctor should discuss decreasing the dose of a statin drug such
as Lipitor or discontinuing the drug. Grapefruit consumption
causes a longer-lasting alteration in the liver enzymes.
Aspirin could cause problems for people taking the bloodthinner Coumadin.
Crestor, a relatively new cholesterol-fighting drug, when
combined with niacin, a B vitamin, can cause moderate to
severe muscle problems.
Sources: Heidi Bragg, drugs.com and Center for Food and Drug Interaction

The profession has come

a long way, Bragg said. We
are required to concentrate on
medicines, so we have a lot of
information about medications
and reactions.
Using only one pharmacy
minimizes the chances of
drug interactions by keeping
all medication records at one
location.
Bragg recommends Vial of
Life, a nonprofit program that
provides a complete picture of all
the medicines a patient is taking.
The list of medications is put in
a medicine bottle which is then
stored in a home refrigerator or

in an automobile for easy access

in an emergency.
The Web site vialoflife.com
provides a printable guide for
listing medications and a sticker
to post in a vehicle or refrigerator
door. Bragg said area emergency
medical officials have been
trained to look for the sticker
when they enter the home of an
elderly patient.
Another site, drugs.com,
offers a drug interaction checker
where you can provide the name
of a drug to see how it interacts
with other medicines you take.
linda.gilchriest@chron.com

Houston

A prescription drug when

taken correctly can save a life,
destroy an infection or minimize
pain. But without knowing how
and when to take it, the medicine
can do more harm than good.
Heidi Bragg, a pharmacist
and University of Houston
School of Pharmacy professor,
said that many times patients
dont provide doctors all the
information needed to properly
prescribe drugs.
A lot of people assume that if
you are taking over-the-counter
medication, it doesnt count,
Bragg said. But doctors need to
have the whole picture, not just
what else you have a prescription
for, but what vitamins and herbs
and non-prescription drugs you
are taking.
For example, the herb St.
Johns Wort grabbed headlines
a few years ago as a natural
antidepressant. What many
people didnt know, Bragg said, is
that St. Johns Wort could not be
taken with Vioxx, a popular antiinflammatory that was taken
off the market in 2004. The
serotonin levels were dangerous
for some, Bragg said.
Bragg recommends two
actions to help take control of
your own health: keep a list of
every prescription, supplement
and over-the-counter medicine
you take, and use one pharmacy
to fill all your prescriptions.
The pharmacy professor said
drug interactions are especially
significant for elderly patients
who often take more medicines.
Bragg said she met with one
woman who took four synthetic
thyroid medications at one
time all prescribed by different
doctors, all different dosages and
all filled at different pharmacies.
She could have died, Bragg
said.
People need to be more
pro-active about their health
care. But that is a lot of
information.
That is where a pharmacist
can step in. Bragg said people
need to get to know the
pharmacist where they get
their prescriptions filled. If the
pharmacist doesnt have time
to visit with a customer when
he/she brings a prescription in
to be filled, the customer should
make an appointment so the
pharmacist can get a complete
picture of all medications,
vitamins and over-the-counter
medications.

WORKOUT DVDIVA

THE ABS DIET FOR WOMEN WORKOUT

Cast: Jessica Smith and two other young things
Bottom line: 55 minutes; Abs, cardio, resistance and flexibility;
advanced beginning-advanced intermediate; 3-pound or 5-pound
weights and a mat (yes, floor work abounds).
Whats cool: Abs hurt when you laugh two days later. This smart
mix of work neednt be done all at once, but youll feel holy if you
do. The cardio baseline is jogging and at interval heights the superfit ladies casually defy gravity. One exerciser stays low-impact and
earth-bound (thank heavens).
What left me cold: The ab-centric Pilates and all too quick yoga
stretches seem afterthoughts. Best if you know those moves
before DVD runs you quickly through the paces that or master
the pause button while doing warrior two.
Buy it: $12.99 at amazon.com

GRADE AFAST & FIRM! 4 REALLY BIG MILES

Cast: Leslie Sansone with mixed age walkers
Bottom line: 56 minutes; Cardio
with a little resistance and flexibility; basic beginningintermediate; stretch band.
Whats cool: Walk, lift knees and kick around Memorial Park+
in your living room with chatty, chirpy but clear direction. Miles
quicken to a pony-tail whipping 10 minutes. Boosted walking
is euphemism for jogging in place and its the interval upper
limit. Endure and youll deserve the nice stretch.
What left me cold: Music is lame and at one point seems
simply sped up a la Alvin and the Chipmunks. Pretending to
do an ab crunch while standing fools no one, no matter what
flashes on the screen about your tummy.
Buy it: $14.99 at amazon.com; $19.95 at lesliesansone.com
