DENPLAN SURVEY PRIZE DRAW TERMS & CONDITIONS

1. By entering this prize draw you agree to be bound by these terms

and conditions and by any other requirements set out in any related
Promotional Material.
2. This prize draw is open to all respondents to the survey excluding
employees of any companies within the AXA Group and their
associated, affiliated or subsidiary companies, and their families,
agents, or anyone connected involved in the administration of this
prize draw.
3. The commencement date of the survey shall be Friday 5th March
2010.
4. The closing time and date of the survey shall be midnight on Friday
24th March 2010 and all entries must be received by this time.
5. All entrants must be over 18 years of age.
6. The first five entries drawn at random by InternetMR, the research
agency managing the survey on behalf of Denplan, shall be the
winners.
7. InternetMR’s decision is final and legally binding on all entrants. No
correspondence will be entered into. Entries that do not comply in
full with these terms and conditions will be disqualified.
8. InternetMR reserve the right to substitute the prize (or any part of
it) for a prize of equivalent or greater monetary value if this is
necessary for reasons beyond its control. There will be no cash
alternative and the prize is not transferable.
9. The winners will be selected at random on Friday 2nd April and will
be informed by email within 14 days with details on how to claim
their prize. If the prize is not claimed within 3 weeks we will again
notify the winner by email. If after six weeks the prize remains
unclaimed the value of the prize will be donated to a registered
charity (Dentaid). Claims more than six weeks late therefore cannot
be paid.
10.The details of the Winners will be made available by Denplan and
can be obtained by writing to 2010 Employee Benefit Survey Prize
Draw, Denplan Court, Victoria Road, Winchester, SO23 7RG.
11.The organiser of this prize draw is InternetMR. The organiser’s
registered address is: InternetMR, Orwell House, 50 High Street,
Hungerford, Berkshire, RG17 0NE.
12.By entering this prize draw, you agree that the organiser is not
liable for any loss, damage or personal injury whatsoever suffered
or sustained in connection with this prize draw (including but not
limited to direct, indirect or consequential losses, loss of profit,
income or goodwill), the use of any prize or any act or omission
(whether negligent or not) of the Promoter or its affiliates, save for
any liability which cannot be excluded by law (including fraud, gross
negligence or for death or personal injury caused by the Promoter's
or its affiliates’ negligence).
13.There will be no charge for entering the prize draw.
14.The Terms & Conditions of this prize draw are governed by English
law and subject to the exclusive jurisdiction of the courts of England
and Wales.
