Brief description of incident:
Loose and exposed electrical wire were hanging down near the water cooler at tank-6 hot end
(IS m/c- 66) hence called and shown Mr. O.P.Roy, the general electrical in charge, physically for
doing his needful. For this short circuit occurred which leads to an accident but nearly missed for
the visitors who was there for filling up his bottle.

Do you suggest any control measures to prevent such recurrence?


On calling Mr. O.P.Roy, the representative from Electrical department, he immediately chocked
the exposed wire and trayed in a proper way. No chance of short circuit occurred till date.

The staircase on the way to first floor of Production floor observed unrailed to hold the railing
leading to which a near miss incident. A worker at that time was coming down and slipped,
which was leading to a fatal injury but a narrow escape as I was standing to hold him up and he
just faces 2 stair rolled, swelling of elbow and just a forehead cut. Lucky escape for him.

Do you suggest any control measures to prevent such recurrence:


On account of that thing immediately called for Mr. Sarkar & Mr. Santunu Banerjee. They visit
the site and immediately called the project team to fabricate on hand railing for the staircase and
the project was completed on the same day. There was no chance of recurrence of the same
activity after that.

Brief description of incident:


One forklift driver was speeding in the production shop floor. Suddenly with a coming of one
person along with a hand trolley, he braked and with that the vehicle skipped and hit the nearby
column leading to a minor injury of forehead swelling of the driver. This accident can leads to
fatal death of two persons. Thats why it is a near miss incident of the month June2012

Do you suggest any control measures to prevent such re-occurrence:


Calling Mr. S. Ganguly (in charge of Forklift maintenance and trainer for forklift driver) and Mr.
Debashis Das (Manager-IR) on the spot - resolve the near miss accident. A periodical training
was planned to provide awareness to the entire forklift driver and guide them about their driving.
We also control the speed limit up to a certain point for not taking any chances to repeat the
occurrence of that incident.

Brief description of incident:


Two persons were engaged for A/C maintenance in the Instrument department. Suddenly a bottle
which was placed nearby fall on ones head leading of swelling and painful reaction on the head.
This incident can leads to hospitalization or sudden shocking death. Further on investigation it
was found that the bottle was placed by them only for drinking purpose.

Do you suggest any control measures to prevent such re-occurrence:


A mass awareness site program was arranged for dealing with this kind of unsafe act.
Call out the contractor people, who were working on the base and held the program.

Brief description of incident:


Non wearing of safety shoes in QA& Cold End departmental people. Cut by glass leads to stop
less bleeding as the cut was deep enough. It was a near miss incident because just ahead of which
was a just scrapped semi-molten form of bottle.
Immediately called for ambulance and first aid provided and send to nearby hospital.

Do you suggest any control measures to prevent such re-occurrence:


Training and PPE compliance audit was planned after that and monitoring of usage of PPEs are
given in top priority.

Brief description of incident:
It was the shrink machine where fire broke out due to sudden power cut and short circuit of the
connection. The fire was ignited slowly and steadily and suddenly a heap of shrink films come
into the way of the fire. The mishap was going to create a heavy damages from lose of resource
point of view. But due to presence of extinguishers in the nearby area, the fire came under
control.

Do you suggest any control measures to prevent such re-occurrence:


Immediately the connections was tripped and disconnects the short circuit area and remend the
connection by Electrical in charge (Mr. Suvra Kanti Roy) and Firefighting program arranged
periodically to get rid of this type of problem in the future.

Brief description of incident:
Three workers were not wearing either helmet or safety shoes during their working time at
height. Suddenly a cement block, medium size, from the top floor come and drawn at the middle
of them leading to a near miss incident. Suddenly called for the SHE team with IR and chucked
the people out for not following the norms of the company.

Do you suggest any control measures to prevent such re-occurrence?


The people were made to exit from the company and PPE compliance audit take over at regular
interval of time, hence re-occurrence of that type of incident was missing after that.

Brief description of incident:
FIRE broke out at the pet coke unloading area of Furnace No-2. Pet coke fire was igniting slowly
and steadily. Initially present employees come into rescue with first aid firefighting system, then;
our emergency firefighting crew came into rescue activities in order to get away from losing
resources and man power as 3 numbers employees were stuck there. BY this time huge flame of
fire was produced and we called the nearby fire brigade team to rescue and help. But our
enormous team effort in firefighting came into effect after few times just before reaching FIRE
BRIGADE.

Do you suggest any control measures to prevent such re-occurrence:


All the plant delegates were present and planned to construct a SILO for getting rid of spillages,
hence re-occurrence of that type of incident was never took place from then.

Brief description of incident:
Once due to leakages in the chutes from the ground floor to the furnace was covered with pet
coke spillages, it was a high chance for broke out of fire as pet coke dust catches fire very easily
and it helps other to burn rather than itself burn. Hence we immediately called for the House
keeping team and turn on the process for cleaning it up. Luckily after cleaning it up, the welding
work start and we get rid of it.

Do you suggest any control measures to prevent such re-occurrence:


Immediately the Leakages were repaired and a general safety and firefighting training is planned
periodically to fight for this kind of situation.

Brief description of incident:
One Electrical Panel was full of dirt and pet coke spillages. Suddenly sparks started to visible
and the operator with the Manager standing beside called out for Help as pet coke dust can
catches fire very easily. The panels connection was tripped and the connection of the panel was
terminated to process the cleaning. After the cleaning of the panel came to an end of chances of
bigger accident.

Do you suggest any control measures to prevent such re-occurrence:


Proper House Keeping maintained and preventive maintenance along with the cleanliness is
practiced regularly for an effective result everywhere.

Brief description of incident:
Motor cover missing of all the shrink films motors. This could leads to serious injury as motors
safe guard shield acts in various ways. Called the Electrical Manager and shown this unsafe
condition. The project team came into act and guards the motor.

Do you suggest any control measures to prevent such re-occurrence:


Monitoring of shrink films motors by the electrical department are made into action for
betterment of this unsafe condition.

Brief description of incident:
No issues of Work permit. Worker doing work on fire was not wearing helmets and safety shoes.
Immediately called the safety department for closing their work and after grating permission
from the concerned department, the work should be resume again.
Working at height is a risky factor and apart from that working on fire without safety shoes is
really an offence and dangerous act to perform which can leads to serious injuries any time,
lucky not this time, hence a near miss incident registered.

Do you suggest any control measures to prevent such re-occurrence:


Issuance of Work permit system came into act and deferring this note will leads to cancellation of
contracts rule came into action. From them such incident never had been registered.

Brief description of incident:


One Electrical board was in broken condition, but due to rain and due to rain water the water was
coming in the contact with the exposed switch and due to water stagnant on the roof of the wall,
the dampness observed. On observing that Mr. S. Thakur immediately chucked the board with
the help of one fitter and the line got disconnected. The effected wall gets deplastered and roof
water made drained into a proper channel.

Do you suggest any control measures to prevent such re-occurrence:


Monitoring increases and removal approval of this kind of unused switch board were come into
act. Re-occurrences of this kind of act were not registered by anybody.

Brief description of incident:


On the floor of Tank-1 the packing materials are lying on the shop floor. An employee
unknowing of the facts stepped on the area and drowned luckily no major injury and cut also. On
account of that the House keeping team was called and scrapped the packing materials out of the
shop floor.

Do you suggest any control measures to prevent such re-occurrence:


House Keeping members are increased with one supervisor level monitoring body was also
placed into the structure for betterment of the result. Hence Housekeeping standard increased and
registered of this kind of near miss incident are lowering in number.

Brief description of incident:


The east side window panes of the quality department got broken by the mal driving of one
manual fork lifter and the lot got fallen on the panes and the panes got broken. The broken glass
scattered all around the floors and the table besides. Luckily the person sitting beside had gone
out for a purpose; otherwise, there would be a high chance of a major accident.

Do you suggest any control measures to prevent such re-occurrence:


One iron member at the corner required to be fabricated so that the obstruction ceased by the
member without delivering any impact to the window panes for breaking it down.

the crush roof falls just besides him and the next half was hanging over his head. This was a
serious issues.

Do you suggest any control measures to prevent such re-occurrence:


The false ceiling need to be plastered and joint firmly so that the crack within the joints are not
firmed out and regular maintenance of the same is required.
