DOCUMENTATION
hÅb/greenroom are undertaking digital video and photographic documentation of Hazard MMX
Where possible, the footage and images will be available to all artists to copy, and will be held by hÅb/greenroom
and used for promotional and archive purposes, which may include a web-based video database.
It will not be used for any commercial purposes.

PERMISSION TO DOCUMENT
By submitting this form, you authorise hÅb/greenroom and its agents to make a digital video and photographic
recording of the work listed above if it is selected for inclusion in Hazard MMX:
You agree that hÅb/greenroom:
 Will, where possible, make this recording available to me to copy;
 Will hold a copy in archive;
 May use the images and recording in full or in edited form for educational, documentation, promotional
and other non-commercial purposes; such purposes may include publication within a video based
website and future marketing materials;
 The digital video recording may also be consulted by the public at greenroom; it may also be used in
educational presentations.
