The undersigned prosecutor states on information and belief that the defendant, either
directly or as a party, at County of Davis, State of Utah, committed the crime(s) of:
COUNT 1
ENDANGERMENT OF CHILD OR VULNERABLE ADULT THIRD DEGREE, a
Third Degree Felony, in violation of Utah Code Ann. 76-5-112.5(2)(a), as follows: That the
above named defendant, on or about June 4, 2016, did knowingly or intentionally cause or permit
a child or a vulnerable adult to be exposed to, inhale, ingest, or have contact with a controlled
substance, chemical substance, or drug paraphernalia.
COUNT 2
ENDANGERMENT OF CHILD OR ELDER ADULT, a Third Degree Felony, in
violation of Utah Code Ann. 76-5-112.5, as follows: That the above named defendant, on or
about June 4, 2016, did knowingly or intentionally cause or permit a child or a vulnerable adult

to be exposed to, ingest, inhale, or to have contact with a controlled substance, chemical
substance, or drug paraphernalia.
COUNT 3
ENDANGERMENT OF CHILD OR ELDER ADULT, a Third Degree Felony, in
violation of Utah Code Ann. 76-5-112.5, as follows: That the above named defendant, on or
about June 4, 2016, did knowingly or intentionally cause or permit a child or a vulnerable adult
to be exposed to, ingest, inhale, or to have contact with a controlled substance, chemical
substance, or drug paraphernalia.
COUNT 4
ENDANGERMENT OF CHILD OR ELDER ADULT, a Third Degree Felony, in
violation of Utah Code Ann. 76-5-112.5, as follows: That the above named defendant, on or
about June 4, 2016, did knowingly or intentionally cause or permit a child or a vulnerable adult
to be exposed to, ingest, inhale, or to have contact with a controlled substance, chemical
substance, or drug paraphernalia.
COUNT 5
ENDANGERMENT OF CHILD OR ELDER ADULT, a Third Degree Felony, in
violation of Utah Code Ann. 76-5-112.5, as follows: That the above named defendant, on or
about June 4, 2016, did knowingly or intentionally cause or permit a child or a vulnerable adult
to be exposed to, ingest, inhale, or to have contact with a controlled substance, chemical
substance, or drug paraphernalia.
COUNT 6
ENDANGERMENT OF CHILD OR ELDER ADULT, a Third Degree Felony, in
violation of Utah Code Ann. 76-5-112.5, as follows: That the above named defendant, on or

about June 4, 2016, did knowingly or intentionally cause or permit a child or a vulnerable adult
to be exposed to, ingest, inhale, or to have contact with a controlled substance, chemical
substance, or drug paraphernalia.
COUNT 7
ENDANGERMENT OF CHILD OR ELDER ADULT, a Third Degree Felony, in
violation of Utah Code Ann. 76-5-112.5, as follows: That the above named defendant, on or
about June 4, 2016, did knowingly or intentionally cause or permit a child or a vulnerable adult
to be exposed to, ingest, inhale, or to have contact with a controlled substance, chemical
substance, or drug paraphernalia.
COUNT 8
SELL, OFFER, FURNISH ALC PRODUCT TO A MINOR - KNOWINGLY, a
Class A Misdemeanor, in violation of Utah Code Ann. 32B-4-403(2)(b), as follows: That the
above named defendant, on or about June 4, 2016, did furnish an alcoholic product to a minor
knowing that the recipient of the alcoholic product was a minor.
COUNT 9
POSSESSION OR USE OF A CONTROLLED SUBSTANCE, a Class B
Misdemeanor, in violation of Utah Code Ann. 58-37-8(2)(a)(i), as follows: That the above
named defendant, on or about June 4, 2016, did knowingly and intentionally possess or use an
unauthorized controlled substance analog or controlled substance, to wit: Xanax (Alprazolam).
This Information is based on evidence obtained from witness Joshua T Lynch.
PROBABLE CAUSE STATEMENT: The undersigned prosecutor is a Deputy
Davis County Attorney and has received information from the investigating officer, Joshua T.

Lynch of the Layton Police Department, and the information herein is based upon such personal
observations and investigation of said officer.
On June 4, 2016, police officers from Layton PD made contact with the defendant
at a residence in Layton. The defendant was under the influence of drugs and had passed out in
front of the home.
Officers entered the home after learning that there were children in the home.
Officers encountered four young males, all under 18 years of age. Those males were found in
close proximity to beer, marijuana, drug paraphernalia, and Xanax.
One of the males stated that the defendant had provided alcohol by setting up a
"beer pong" game with the males. Another male advised that he had obtained Xanax and beer
from the defendant, and that the males had found the marijuana inside the home. A third male
advised that he had gone with the defendant to Ogden to purchase the Xanax.
Officers also located three young children in a separate room in the home.
The defendant was interviewed and she admitted that she had obtained Xanax and
did not have a prescription for that controlled substance.
