Write them a letter or note.

Write your full name, year & section on the recommendation form.

Inform Ms. Mitch who will be recommending you on or before the set due date.

Provide a white long letter envelope, and profile card to Ms. Mitch on or before the set due date.

Give your teachers 2 weeks to finish your recommendation forms.

Ms. Mitch will forward the recommendation forms to the teacher s concerned and collect them.

Take note of the due date and meet the

deadlines.
