BPLO (or Treasurer if LGU has no BPLO) as the preparer/main respondent of the form and
Administrator (or Mayor) for purposes of concurrence and endorsement.
This report aims to capture baseline data on important indicators for measuring
performance of the BPLS reform program and other information relevant to the BPLS
reform process. This report shall be accomplished only once by each LGU.
Instructions for accomplishing selected entries in the form
Explanation
This captures status of compliance by LGU to the BPLS standards prescribed by DTI-DILG
Joint Memorandum Circular 01. Indicate actual LGU compliance per BPLS parameter
(namely: number of forms, number of signatories, processing time and number of steps)
before and after the BPLS reform process. If LGU is fully compliant with the four BPLS
standards, state month and date full compliance was achieved by LGU. If non-compliant in
any of the four BPLS standards, indicate reasons for non-compliance despite BPLS
streamlining initiatives
This captures data about LGU actions or initiatives that are supportive of adoption and
institutionalization of BPLS reform. Indicate Y in the column Status if initiative has
been done or being done by the LGU or N if not yet done. As to the issuance of legal
framework, specify type of legal instrument, e.g., Executive Order, SB ordinance, etc.
Indicate number of new business registrations, business registration renewals and closed
businesses per year from 2009 to 2011. Also indicate percent of total registered business
establishments under the category of single proprietorships (i.e., excluding
corporations, partnerships and cooperatives) owned by or registered in the name of a
woman.
Indicate amount of revenues (in pesos) generated by the LGU from mayors permit (or
local business permit) fees, regulatory fees and local business taxes per year from 2009 to
2011. Revenue data may need to be obtained from or validated by the Treasurers Office.
Indicate number of jobs generated from new business establishments and renewed
business establishments per year from 2009 to 2011.
Indicate estimated amount of investments from new business registrations and estimated
gross sales from renewed business establishments per year from 2009 to 2011.
Indicate number of establishments inspected by the Joint Inspection Team (JIT) organized
by LGU in support of BPLS reform per year from 2009 to 2011.
List down major problems and issues encountered during implementation of BPLS
streamlining and actions taken by LGU to address each problem/issue.
Indicate type and source of any external assistance received by the LGU in implementing
BPLS streamlining such as training of BPLO personnel, technical advice from BPLS coaches,
etc.
Attach copies of official documents related to the BPLS streamlining initiatives of the LGU
such as EO authorizing the BPLS reform process, streamlined BPLS flowchart, etc.
