INITIATIVES

INVENTORY QUESTIONNAIRE
This data gathering instrument forms part of the DOST-PCIERRD-funded Project: REGIONAL DISASTER SCIENCE AND
MANAGEMENT.
The project aims to conduct an inventory of the various Disaster Risk Reduction and Climate Change Advocacies as part of the
SUC-led surveillance activity. This instrument will be disseminated and filled out by LGUs at the municipal levels. The yielded
inventory will be integrated in a centralized web-based information system in the region which will become a source of DRR and
CCA documents available for policy makers, planners and development workers for future activities in relation to disaster risk
reduction and climate change mitigation and adaptation efforts in the country.
All the information that you shall provide will be treated with confidentiality and shall be used only for the study being undertaken.
NOTE: Information asked for by this inventory questionnaire should constitute data and information from the last five
years (2008-2012).

Questionnaire Content
This data gathering instrument is divided into five sections. Highlights of each section are given as follows:
Part 1 (Questionnaire Information) contains the Respondent ID No., as well as the location and date of filling out as well as the
name of the person who filled out the questionnaire.
Part 2 (Profile) refers to the personal circumstances of the key informant.
Part 3 (Inventory of DRR & CCA Studies) is a data field which deals with the listing of the various Disaster Risk Reduction and
Climate Change- Related Studies implemented in the community. The data field speaks of the various LGU-initiated researches,
SUC-initiated R&D projects and or internationally-funded research and development activities related to DRR and CCA.
Part 4 (Inventory of Existing Hazards Maps) is a data field which deals with the listing of various Existing Hazards Maps in the
community.
Part 5 ( Inventory of DRR and CCA-Related Capability Building Activities) is a data field which deals with the listing of DRR
and CCA-related Capability Building Activities.
Part 6 (Inventory of Multi-hazard Early Warning Systems) is a data field which deals with the listing of Existing Multi-hazard
Warning Systems implemented/available in the community.
Part 7 (Networks and Linkages) puts focus on the various networks and linkages established by community in relation to DRR
and CCA-related activities.
Part 8 (Problems and Issues) concentrates in the identification of issues and concerns dealt with by the community related to
DRR and CCA.
