Supported Housing - provides an array of housing options ranging from
24 hour awake supervision to independent unsupervised settings. Supported Employment - provides employment assistance and support for individuals with significant mental health diagnoses for whom competitive employment has been interrupted or intermittent. Support services include job placement, job coaching, and crisis intervention so that consumers can maintain part or full-time employment; Assertive Community Treatment (ACT) - provides intensive, integrated community-based mental health intervention and support services designed to reduce psychiatric hospitalizations and increased community tenure Integrated Care - clinical consultation, case management, and supportive services that assist individuals transitioning from psychiatric hospitals and nursing homes to the community-based behavioral health system.
