Troutt, Chief Boller Inspector, Texas Department of Licensing and Regulation (TDL) received notice of an ongoing Carbon Monoxide (CO) accident at Marshall Middle School, 6455 Gladys Ave., Beaumont, Texas 77705 (’Marshall"). Marshall is part of the Beaumont Independent School District (BISD). The reported accident occurred at approximately 10:15 a.m. on Thursday, January 28, 2016. On te same day a preliminary investigation was conducted. The source of the CO was suspected to be from the boller mechanical room. For a history of the equipment in the mechanical room, see Exhibit 1, ‘According to Mr. Greg Lookabaught (BISD Director of Maintenance), there were no fatalities related to this accident; however, one hundred seventy-five (175) people (students and teachers) were treated by Emergency Medical Services (EMS) on site for carbon monoxide exposure. Siy-nine (69) of those treated on site were transported ta local medical facilities. It \was estimated that another one hundred five (105) went to local medical facilities as walk-in patients TDLR Deputy Boiler Inspector, Richard Charland was immediately dispatched to Marshall from Houston, Texas. TDLR Chief Boiler Inspector started travel to the location from Austin Texas. Upon arrival, Mr. Charlane contacted Mr. Greg Lookabaught and Mr. Dean Moore (BISD Maintenance Technician). At that time, evacuation ofthe students and teachers was complete. ‘The firefighters and Emergency Medical Services (EMS) concluded work and left the location. It ‘was reported to Mr. Charland, (Exhibit 6) that firefighters had identified the source of the CO to. be from the boiler mechanical room. It was also reported to Mr. Charland that both boilers ‘were turned off by school maintenance atthe direction of the Fire Department. Mr. Charland proceeded to the boiler room, utilizing his state issued gas analyzer to check CO levels along the way as well as in the boiler mechanical room (readings are 0). Upos entering. the boiler mechanical room Mr. Charland observed two (2) boilers, TX160840 (used for Hot Water Heating) and 71192254 (used for Hot Water Supply), Mr, Charland noticed soct bulld up, ily Arment, CB ‘Tomar Cu ar Teer ‘is gen ~ Mid eet ‘Fr Newer” lam, Togs Marshall Middle School 6455 Gladys Ave Beaumont, Texas 77706 Re: Boller Accident Investigation on TX160840 ‘on both boilers. 11192254 had indications of "Flame Rollout” from the bummer and excessive soot build up in the contrat panel and in the flue pipe. TX160840 had minor soot build up in the bottom right side of boiler and on the right header, no soot build up was noted in the flue pipe. (Mr. Charland reported his findings to me while | was stil in route. | instructed Mr. Charland to. gain access to the roof to check the flue pipes of both boilers for obstructions, Mr. Charland later reported no obstructions were found. When | arrived at Marshall, | met Mr. Lookebaught, Me. Moore, Mr. George Luke (8150: ‘Assistant Director of Maintenance) Mr. Mark McClelland (81SD Maintenance Supervisor). | requested Mr. Lookabaught give a verbal report of the day’s events with regard to how the: problem was determined to be CO as well as how it was traced back to the boiler mechanical room. Mr. Lookabaught reported that at approsimately 10:15 a.m, the school principal called 911 due to multiple students being sick at the same time. When Emergency Medical Services (EMs) personnel arrived, the personal CO Detectors (carried by EMS) immediately started going. into alarm. When the fire department arrived, they walked the halls of the school with a 4 Gas. ‘Analyzer which lead them to the Boiler Mechanical Room. Firefighters instructed the bollers to be secured at that time, (On examination of the boilers, {noticed the same condition of the boilers as observed by Mr. Charland. | also noticed there was an air handler installed in the boiler mechanical room. The inet side ofthe air handler had a fiter rack installed which did not have the cover installed. The air handler moved the CO to the Library and adjacent hall way through the attached ductwork. From there, other air handlers in the school picked up and dispersed the CO throughout the school. While conducting this part of the investigation, Beaumont Fire Marshall Joseph B Condina Il arrived on location to meet with me. While speaking with Mr. Condina, | asked if the Firefighter who walked the halls of the school with a gas analyzer could come back to the school to discuss his findings during the incident. (Mr. Codina agreed and dispatched Firefighter John Butcher back to the school. After meeting Me. Butcher, asked him to walk the route he hed taken when he was looking for the source of CO. While walking this route, Mr. Butcher was verbally reporting the ranges he saw in “parts ‘er milion” (ppm) on the analyzer. It should be noted, the readings were not logged, therefore the reported reading were recalled from Mr. Butcher's memory. He stated at the entry of the school he was reading approximately 150 ppm, as Mr. Butcher walked down the fist hall on the right (towards the library) the reading were increasing to approximately 400 ppm. Mr. Butcher Worked his way to the boiler mechanical room. When opening the door, the reading spiked to ‘over 400 ppm. It at this point he felt that he had found the source of CO, he ordered the equipment to be shut down and exited the building, Marshall Middle School 6455 Gladys Ave Beaumont, Texas 77706 joller Accident Investigation on TX160840 | then turned my attention to boilers TX160840 and TX192254, | started with TX192254 as this 's the boiler I suspected to be the cause of the problem. This was due to the excessive soot build up in the flue pipe, and control panel as well as the electrical wring that was burn: from “flame rollout". | asked Mr. Condina if he was ok with me starting boiler TX192254 as long a5 | was taking CO readings, he agreed. Two separate 4 gas analyzers were used, one issued 10 Mr. CCharland from TDLR and the other belonging to Beaumont Fire Department. Boiler 74192258 was put inte operation with one analyzer located on top of the boiler and the other located in the control panel. I watched as the boiler went through Its correct startup cycle. The boiler ran through a complete firing cyle (includes purge, ignition, bringing weter to ‘temperature and shut down) without any problems, the analyzers did not go into alarm. Boiler TX160840 was then tested in the same manner. In less than a minute of the boller’s ‘lame ignition, both CO detectors went into alarm. A flame could also be seen exiting the boller into the Boiler Mechanical foom on the right side of the boller under the header. This was determined to be the source ofthe CO. While running these two test; it was noticed the power ventilator for the Boiler Mechanical Room which is installed above and in front of TX160840 was not operating. Its unknown Ifthe fan was not operating due to failure of the fan or due to power being secured with the ‘occurrence of this accident. Texas Boiler Rule 65.603 (b) requires when a Power Ventlator is used for Boller Room Ventilation the fans shal be sized on the basis of 0.2 cfm for each 2,000 ‘Btu/tir of fuel input forthe combined burners located in the boiler room. The boiler and fans shall be interlocked so that the burners will not operate unless there is a supply of combustion, ventilation and dilution air asrequired by the boiler manufacturer's recommendations. (Mr. Charland red-tagged both boilers (TX192254 and TX160B40). 1X192254 was red-taggsd for excessive soot buildup, damaged wires in the control panel and power ventilator non- ‘operational. And, TX160B40 was red-tagged for lame exiting the side of the boller and power ventilator not operating. January 29, 2016-1 went back to Marshall. Upon arival| met with Mr. Lookabaught, Mr. Luke, ‘Mr. McClelland and Mr. Calvin Timmons (RayPak Boiler Representative who was called by BISD). When entering the Boller Mechanical Room, Mr. MeColland informed me the room was equipped with two power ventilators instead of the one which was noted on January 28, 2016, Review of the buildings blueprints (Exhibit 7 & 7-1) obtained from ISD show one fan was used {or combustion ar (push ar into the room) and the other used for exhaust (push ar out of the Marshall Middle Schoo! 6455 Gladys Ave Beaumont, Texas 77706 Re: Boller Accident Investigation on TX160840 room). The exhaust fan is the fan noted as not operating on January 28, 2016. The exhaust fan was removed and is in the process of being replaced, | removed the Red Tag on boiler ‘74192254, as the required repairs to the boiler were complete and the exhaust fan was in the process of being replaced. | requested a report from Mr. Lookabaught regarding the ‘operational status of both the combustion alr fan and the exhaust fan (Exhibit 8). While on site 1 made 2 video (exhibit 9) of boler TX160840 while the boller was turned of ‘Then another video (exhibit 10) of TX160840 with the boiler running showing the fame exiting the side ofthe boiler. February 1, 2016 - I received a text message from Mr. Timmons which contained picture of a hole that was knocked into the left side of boiler TX160840 firebox (exhibit 13) It was later determined this hole was made by personnel conducting asbestos sampling in preparation for the boilers replacement. March 2, 2016 - A meeting was held at Marshall with James P, White (General Adjuster for CNA Insurance), Kennetha W. Lucas (Attorney representing CNA Insurance), John R. Puskar (Professional Engineer representing CNA Insurance), Gregory T. Barnhill (Executive General ‘Adjuster for Crawford Global Technical Services representing Lexington Insurance) and Patrick Campion (Senior Technical Consultant for PT&C/LWG representing Lexington Insurance). During this vist, Mr. Puskar and Mr. Campion were shown exhibits 9 & 10. During the site visit, Mr. Puskar, Mr. Campion and | attempted to remove one of the burners for boiler TX160840 without success. It was during this time it was noted the right side of the firebox frame had signs of excessive deterioration when compared to the lft side of the boil ‘The deterioration was to both the metal frame and the refractory ofthe fire box. Two samples Of refractory from boiler TK160840 (these samples were entered into TDLR Enforcement Evidence Locker upon my return to TDLR-North Campus on March 4, 2016) were taken. The dot where the source of water was coming from, my experience leads me to suspect one (1) of following three (3) instances occurred. 1) A water leak in the heat-exchanger (which would be proven by a liquid pressure test). 2) Condensate created in the boiler flue pipe. 3) Water leaks from the building roof (excessive water tracks on the exterior of the boiler stack (Exhibit 12 through 12-2) were noted). A Liquid Pressure Test of boiler TX160840 heat-exchanger was scheduled for March 29, 2016. ion appears to be caused from water entering in the firebox. While it was not known March 29, 2016 - Mr. Puskar, Mr. Barnill, Mr. McClelland, Mr. Joseph Kennedy (with PT&C/LWG) and | met at Marshal to conduct the Liquid Pressure Test of bollers TX160840 heat Marshall Middle School (6455 Gladys ave Beaumont, Texas 77706 Re: Boiler Accident Investigation on TX160840 ‘exchanger. The procedure (exhibit 13) drafted by Mr. Puskar was followed. No leaks were found (Exhibit 24). During the Liquid Pressure Test, all bumers on TKIG0B40 were removed. This allowed photographs (Exhibit 15 through 15-4) of the boilers Firebox to be taken, These: photographs confirm the excessive corrosion of the refractory and metal frame which holds the: bburmers inthe fire box. The excessive corrosion was only located on the right side of the boiler. The investigation was concluded and the boller TX160840 was released to the school for replacement or repalt. Observations: The boiler was inspected Just two months prior to the accident occurring. it Is my belief Mr. Nester could not have identified the CO leak unless he was utllzing a gas analyzer that detects CO. It is unknown if the boiler was leaking CO at the time of the inspection as there are no ‘obvious indications. Since CO is an odorless colorless gas and therefore cannot be detected ‘without a CO detector and Texas Boiler Law and Rules da not require boiler inspectors to test air quality in the boiler rooms, the inspector did not test for CO. Mr. Nester would have looked at the area where the flame was exiting the boiler during his inspection. However, when ‘conducting this part of the inspection the boiler would not be operating as there would be 2 burn hazard to the inspector, therefore the flame would not be visible. I noted the fiter cover ofthe air handler was not installed. While | was able to determine this is the most likely avenue for the CO to gain access from the Boller Mechanical Room to the school; | am also aware had this cover been installed, it isnot airtight. The CO would have entered the air handler anyway. By this cover not being installed, the CO would be pulled into the school via the air handler more rapidly than ifthe cover was installed Since there was aflame exiting the bolle fre box through the side, there is an obvious failure fn the boiler. Examination of the boiler showed corroded refractory and the metal burmer rack located on the right side of the boiler fire box which isthe same side of the boiler from which the flame was escaping. ‘There were two (2} Power Ventilators (one Combustion and one Exhaust) installed inthe Boiler Mechanical Room (Texas Boiler Rule 65.602 (b) requires power ventilator to be interlocked with the burners of the boilers to prevent operation if there is not adequate air supply), The ‘combustion air fan was non-operational and the exhaust fan was operational. With the exhaust fan pulling air out of the boiler room without air supplied by the combustion air fan, the boiler oom would be under a negative pressure, With the boiler room under a negative pressure, air from the outside would be pulled in through the flue pipes of both bollers (TX160840 and 1192254). It should be noted this theory could have been proven by operating, the boiler after both fans were operating as designed. However, since a hole was made in the. ‘side of boiler to check for asbestos (without approval by TDL), testing could not be performed, Conclusion: Based on the evidence collected, this accident occurred as a result of two different failures, First was the non-operational status of the power ventilation fans. The second is due to the flame that was observed exiting the side of the boiler which was caused fiom corrosion of the fire box refractory. Prevention: While itis this investigator's opinion, this accident could have been prevented by having @ Carbon Monoxide Detection or Alarm system installed in the boiler room, This system would have tobe installed in one of two methods as listed below or both 1. Have a CO monitor installed in the boller room. The CO monitor would be of @ manual reset type and would be interlocked with all gas appliances in the room in such a way a5 to stop their operation if the CO monitor went into alarm or lost power. 2. Have @ CO monitor installed in the boiler room. The CO moniter would be installed In such a manner that an alarm would sound throughout tte schoo! letting all occupants know of the hazard. In either case, B1SD should consult with the local fire department, city building officials and a Combustion Engineer to determine the best method of prevention for this location 6-2-8 yy. Boller TX160840 -was manufactured in 2013 and "H” stamped in conformance with ASME Pressure Vessel Code, Section 1V, 1986 Editen 1987 Addenda (Exhibit 2). Equipment information sas follows: 1 Boiler Tx160840: Type: Water Tube ‘Manufacturers Serial Number: 888104775 National Board Number: 68175 Year Bul: 1988 ‘Maximum Allowable Working Pressure: 160 psig penge 2 Safety Vanes: a. Single safety valve configuration located on the outlet header, set pressure 125 Pig, relieving capacity 2,070,000 btu. The EMTs carry CO ‘monitors and upon entry started going off on indication of CO. Thisis when the evacuation occurred. “The Haz- Mat team arived with CO monitors that give actual readings. School entry Was tested and it showed a reading 150-200ppm. This boler had signs of flame rollout with back sot and melted wires. The Beaumont Fire Department had the CO manitr in thelr possession and the CO monitr alerted the team to an elevated level of CO. ISD immediately secured boller operations to the domestic hot water boller and the bung heating bole. At that point building ventilation was underway. [At approximately 5:30pm January 28,2016 state bole inspector arrives on-ste. January 28,2016 the state boler inspector red tagged both boilers. The building hot water boiler was red tagged because of CO concerns. The domestic hot water boiler was re tagged because the vent fan above the bulding hot water boller was not operating at tat time, Possibly due to power switches not on, The state baller inspector told BISD he would remove the red tag on the domestic hot water boiler once the vent fan above the building hot water boiler was operational. January 28,2016 BISO staff energized the vent fan above the building heating boller. However the vent fan was making 2 noize, BIS0 made the decision to replace the vent fan above the bulding heating boiler.
