Waiver, Consent and Release of Liability

WARNING: READ CAREFULLY. THIS AGREEMENT INCLUDES A RELEASE OF LIABILITY AND WAIVER OF LEGAL RIGHTS AND
DEPRIVES YOU OF THE RIGHT TO SUE THIS ORGANIZATION AND OTHER PARTIES. DO NOT SIGN THIS AGREEMENT UNLESS
YOU HAVE READ IT IN ITS ENTIRETY. SEEK THE ADVICE OF LEGAL COUNSEL IF YOU ARE UNSURE OF ITS EFFECT.
Admission of Risk and Liability Release
I am applying for racing membership and/or an officiating membership in American Bicycle Racing, Inc. I understand and accept the fact
that racing a bicycle can be dangerous and I fully accept the risk that may be involved. I wish to forever release American Bicycle Racing,
Inc., its clubs, employees, members, promoters, and sponsors from any liability due to any accidents in bicycle races involving American
Bicycle Racing, Inc. I presently do not know of any mental or physical condition, which would affect my ability to participate in bicycle
races.
FOR MINORS: Parent or Guardian must agree to this waiver.
I am the parent or guardian of the above listed Applicant, and assure American Bicycle Racing, Inc. that the facts listed above concerning
my child or ward are true. By signing this form I am giving my permission for my child or ward to enter any bicycle race or event
sanctioned by American Bicycle Racing, Inc. during the period of the license applied for and also agree to the terms of the above listed
Admission of Risk and Liability Release.

I understand that this Waiver and Release may be stored electronically and agree that a copy is authentic and admissible as evidence in
any future dispute or proceedings.
