Sir/Madam,
Please refer your two applications dated 24/10/2016 (received in this office on
25/10/2016 and 26/10/2016) seeking information under the RTI Act-2005.

The;,.documents required for issuing a passport to a member from the transgender


community are:
(a) A Sworn Affidavit regarding the change in sex.
(b) Supporting certificate from either the concerned surgeon or the government/private
hospital where he/she underwent the medical treatment for the purpose or in its absence
medicai examination from Chief Medical Officer of Government Hospital.
(c) Any two identity documents bearing the new name and gender.
(d) Since the change in sex will also normally require a change in name, the applicant in
that eventuality, will also have to follow the procedure regarding change of name as prescribed
by the passport office.

As requested by you, please find herewith enclosed certified copies of page 10, 11 & 12
from instructions sheet carrying the available document list.

Aadhaar Letter/ Card (Aadhaar letter/card or the e-Aadhaar (an electronically generated letter from the website of

Note:

I i) Applicants are required to submit the proof of address of the present address only, irrespective of the date from
, which he/she has been residing at the given address. However, he/she is reqUired to mention all the place of stay
during previous one year in the Passport application form.
ii) Any of the remaining documents containing address out of sixteen documents listed under Tatkaal application,
, could also be accepted as proof of residence if such documents have the same present residential address as given
by the applicant in the Passport Application Form.
iii) FurnishinQ of Aadhaar card will expedite processinQ of passport applications.
Proof of Date of Birth. For Proof of Date of Birth attach one of the following documents:
For applicants born on or after 26.01.89, oniy Birth Certificate issued by the Municipal Authority or any office
authorized to issue Birth and Death Certificate by the Registrar of Births & Deaths is acceptable.
The Birth Certificate should ordinarily contain the name of child, name of father and mother, date of birth, place of
birth, sex, registration number and date of registration. If the Birth Certificate doesn't contain the name of child, a
declaration on plain paper siQned by parents, is reQuired to be submitted specifying the name of the child.
Birth certificate issued by a Municipal Authority or any office authorized to issue Birth and Death Certificate by the
Registrar of Births & Deaths
School leaving certificate / Secondary School leaving certificate/ Certificate of Recognized Boards from the school last
attended by the applicant or any other recognized educational institution

c.

Affidavit swom before a Magistrate/Notary stating date/place of birth as per the specimen in Annexure "A" by illiterate
or semi-illiterate applicants (Less than 5"'class).
