15. METHOD STATEMENT: (STEP BY STEP PROCEDURE)

BEAM CLAMP PRE-USE EXAMINATION


1. The SWL/WLL is adequate for the load
2. The Colour Coding is current and the clamp has a plant number/ID mark
3. Examine suspension and shackle and check for wear,stretch and distortion
4. Examine outer and inner clamp and check for wear,stretch and distortion
5. Examine load bar and check for wear,stretch and distortion
INSTALLATION OF BEAM CLAMP
6. Where possible,position the beam clamps on the beam flange directly above the load to be lifted (to
avoid/minimize any angular loading)
7. Tightening the clamps using hand only to avoid any over tighten risk that might be causes undue pressure on
the clamp knuckles and can result in the clamp failing
8. Ensure the beam from which the clamp is to be suspended is adequate for the loading.
9. Ensure the hoisting unit is correctly seated in the suspension shackle
10. Ensure the correct clamp is selected to suit the beam flange and the weight of the load to be lifted
CHAIN BLOCK PRE-USE EXAMINATION
11. The SWL/WLL is adequate for the load

14. There are no signs of misuse,i.e. stretched hooks,cracked casings,stretched/distorted chain etc.

INSTALLATION OF CHAIN BLOCK
16. Ensure that the structure from which a hoist is suspended is adequate for the load its intended to carry.
17. The top hook should be fitted with a safety catch

18. After Chain Block installed,ensure that load and hand chain is hang freely and not twisted
19. The Lenght of the load chain should be checked to ensure rhat the bottom hook will reach the lowest point
required without running the load chain fully out.
SAFETY PRE-CAUTIONS OF CHAIN BLOCK
20. Do not overload
21. Do not leave a suspended load unattended
22. Do not stand under the load
23. Do not use a Chain Block as a Sling or choker
24. Maximum Side Loading is 15 degrees
25. Only one operator should pull on a single hand chain at one time
26. Ensure the load chain is properly seated in wheels or sprocket prior lifting
27. Develop, review, and accept Permit-to-Work for lifting operation.
28. Barrier off or police area where lifting operation shall occur as required.
29. Make announcement on public address system prior to commencing lift.
30. Conduct toolbox conversation.
31. Select and conduct pre-use inspection on all rigging equipment to be used in operation.
32. Attach all rigging equipment to the lifting points as per Sketch and material list.

33. Ensure Completion of debrief, comments and lessons learnt section at end of plan.
Guidance: Always bear in mind that the lifting plan could well be used by someone else and therefore needs all the relevant
steps to be recorded.

17. GENERAL NOTES:

1. Pre-use inspection of all equipment and lifting devices shall be done on site.
2. Extreme care shall be exercised throughout the lifting operation to ensure safety to personnel, plant,
environment, and equipment.
3. If for any reason conversation ceases between the Crane Operator and Banks man, all operations shall
cease until re-established.
4. Riggers have the responsibility of ensuring that the correct rating and colour coded lifting and rigging
equipment shall be used for all lifting and rigging operations.
5. Banks man shall always wear high visibility vests during all lifting and rigging operations.
6. If load is not visible to the crane operator (blind lifts) there shall be the use of two (2) or more Banks man
and consideration should be given to communication breakdown during blind lifting.
