1. How do you preserve the security and confidentiality of documents in your laboratory?
- First of all i will assign my staff to organize the documents in a cabinet or i will do it
myself. If a document needs to be disposed, i will use a shredding machine or burn the
documents so no one can read the info in the documents. Filing, sorting or arranging the
document will be done by the assigned staff and no one else. Customer concerns about the
documents shall be notified by the owner or the Head of the clinic. The documents cabinet
shall be locked whenever the staff responsible for it is not present.

2. What are the factors to be considered in designing your laboratory result form?
- The result form lettering should be readable, has a name and signature of the staff who
released the results, should have the basic information of the laboratory ex. address, contact
number, etc.

3. How will you manage your laboratory using the information technology process?
- Information technology process is very usefull in storing and keeping laboratory info. It can
also be used for tracking our inventory and the Turn-Around-Time for calibration.

4. What are the main objectives in managing your own laboratory?


- The first objective is for the staff to provide quality health care to the clients. Second, is to
produce accurate and reliable results to the clients. Third, is to gain profit in order for the
laboratory to grow and avoid bankruptcy.

5. What are the documents design needed for clinical laboratory?


- The analytical plan, data, samples/specimens (where appropriate), analytical results and if
issued the final analytical report, Records of all audits performed by the quality audit
function; the qualifications, training, experience and job descriptions of personnel; and
reports of the maintenance and calibration of equipment, The historical file of standard
operating procedures including the index, plus any operating manuals used as part of an SOP
and The records and results of all the quality control tests performed to confirm the accuracy
of the work.
6. What are the objectives of managing quality patient service
- First objective is to meet the expectations of the clients and satisfy them with our services.
Next, is to avoid errors done in releasing the results or to provide accurate results. Third, is to
make the clients have a pleasant experience in our laboratory so that whenever they needed
to be tested, our laboratory will be their first choice.

7. How is the quality of design elements in the laboratory ensured?


- The floor plan should follow the standard measurement of each section of the laboratory for
the convenience of the staff. The chemicals should be arranged according to the kinds of
classification system to use to be used avoid misplacement and to find the chemicals easily.
The machines should be placed on their sections according to their uses.

8. What are the primary forms of documentation required by the organization?


- Laboratory report of significant findings, Requisition forms, Financial documents,
Transactional documents and reports, Contracts, Business permit, Moas, Emails and
memoranda and letters.
9. How is delivery of quality service in clinical laboratory ensured?
- Delivery of quality service in clinical laboratory is ensured by effective laboratory
management. Planning, Staffing, Budgeting, Inventory management, Financial management
and Technical management are necessary. Client feedback is also necessary for providing
quality service because the feedback is one of the most essential variable for the
improvement of the laboratory.

10. What are the types of business softwares you use in your laboratory set-up? Discuss.
l Reporting software. It is used to generate aggregated views of data to inform the
management about the state of their business.
l Procurement software. A business software that aids automate the purchasing
function of organizations.
l Employee scheduling software. Used for creating and distributing employees
schedules, as well as tracking employees hours.
