1.0 OBJECTIVES

Provide a documented process for recording statistical data of all work related
injuries, occupational diseases, accidents, and incidents in Ras Laffan
Industrial City, (RLIC) in order to implement proactive action plans, which in
turn can assist to prevent future incidents.

2.0 SCOPE

This document and its contents are applicable to all activities and their
respective aspects within the official Ras Laffan Industrial City boundaries,
including the port facilities.
This Procedure sets the minimum requirements for progressive HSE statistics
kept on a monthly basis for RLIC Employees, Contractors and End Users in
RLIC. Qatar Petroleum Corporate employees conducting work activities at
RLIC resides under the QP Corporate HSE system.

4.0 DEFINITIONS, ABBREVIATIONS and GENERAL CONCEPTS


This section contains Definitions, Abbreviations and General Concepts
necessary for the reader to clearly understand the content of the document.
4.1 Definitions

Accident An accident is any event, which has caused


injury or illness. Such an event may also
involve damage, product loss or interruption of
work.
Approve to accept as satisfactory, permit or officially
agree
End User A company, firm, or RLC department that uses
services, facilities, and occupies RLC land for
the purpose of manufacturing a product or
providing a service to RLC or other tenants
within of RLC.
Environment An unplanned event or chain of events that has

Exposure Hours The total number of hours worked including
overtime and training but excluding leave,
sickness and other absences.

equipment, loss of production or impact on
environment.
Note: Every accident is an incident, but not
every incident is an accident.

Near Miss An undesired event or chain of events, which,


under slightly different circumstances could
have resulted in an incident? It affords the
opportunity to investigate and implement
corrective actions to prevent recurrence of a
similar incident with more severe
consequences.

4.3 General Concepts

4.3.1 Occupational Injury:


Any injury such as a cut, fracture, sprain, amputation, etc. which results
from a work accident, or from a single instantaneous exposure in the work
environment

Note: conditions resulting from animal bites, such as insect or


snakebites, and from one-time exposure to chemicals are considered to
be injuries.

4.3.2 Injury Incident Rate:


Injury Incident Frequency Rate (TICF) -The Total Incidents reported
Reportable Cases per 1,000,000 exposure hours.

4.3.3 First Aid Treatment Cases:


First aid treatment cases are one time treatment and subsequent
observation of minor injuries (Scratches, cuts, burns, splinters etc. which
do not ordinarily require medical care) even though provided by a
physician or registered professional personnel.
Note: Record will be kept of first aid treatment cases but because they are
not per definition, recordable cases, they will not be included into injury
rate calculations.

4.3.4 Work related injury:


An injury is considered work related if it results from an event of exposure
in the work environment. The work environment is primarily composed of:

The employers premises, and


Other locations where employees are engaged in work related
activities or are present as a condition of their employment. When an
employee is off the employers premises, work relationship must be
established, when on the premises, this relationship is presumed. The
employers premises encompass the total establishment. This includes
not only the primary facility, but also such areas as company storage
facilities, cafeterias, and rest rooms. In addition to physical locations,
equipment or materials used in the course of an employees work are
also considered part of the employees work environment.

To determine if injuries resulted from traffic accidents should be


included one should always ask the following two questions:

Was the person involved in the accident on official business?


Was the person performing an instruction from his employer?

The following injuries sustained during a vehicle accident should be


regarded as injuries on duty:

When a person is using a vehicle inside or outside Ras Laffan to


perform his/her duties or to carry out an instruction/task given by a
supervisor.

When an employee is under his/her normal duties required to work


from his/her house as a base and to travel by the shortest direct route
from there to various areas to fulfil his/her duty to his/her employer,
he/she is acting in the course of his/her employment.

When an employee is travelling by the shortest direct route in a


vehicle provided by the company to and from courses, seminars or
meetings outside Ras Laffan.

When an employee is travelling in his/her own vehicle to and from a


course, seminar or meeting in his/her private vehicle provided that:;

The employee was instructed by his/her employer to attend the


course, seminar or meeting
No company vehicle was available
The employer compensates the employee for the use of his/her own
vehicle (travel expense claims)
A direct route was used to and from the course, seminar or meeting.

When a person on standby or any other person is called out after normal
working hours to perform a duty at work and is using his/her own private
vehicle it will be regarded a works injury from the moment the person
leaves home until the person arrives back home, provided that the person
is travelling directly from home to work and from work to home.

4.3.5 Contractors not under RLC/HSE supervision


End Users or outside contractors services working under their own
supervision. For example, scaffold builders, contract maintenance teams
during shutdowns, and contractor teams during construction phases of a
project, cleaning services etc. are not be included into RLCs HSE
statistics.

4.3.5. Fatality Cases:


Fatal incidents resulting from occupational injuries (Regardless of the time
between the injury and death)

4.3.6. Lost Work Day Cases:


The number of workdays, (consecutive or not) beyond the day of the
injury, that the Employee was away from work because of an occupational
injury. Cases, other than fatalities, which result in lost workdays.
Any Occupational Injury or Illness which renders the person temporarily
unable to perform their regular job or restricted work on any day after
which the injury or illness was received or diagnosed. In this case any
day includes rest day, scheduled holiday, weekend day, public holiday, or
subsequent day after ceasing employment. A single event can give rise to
several Lost Workday Cases depending on the number of people
affected.

4.3.7. Recordable Cases:


Total Reportable Case Frequency (TRCF) -The Total Reportable Cases
per 1,000,000 exposure hours. All work-related fatalities and those work-
related injuries, which result in:
Losses of consciousness, restriction of work or motion, transfer to another
job, or require medical treatment beyond first aid.
Therefore:
* All fatality cases are Recordable.
* All lost workday cases are Recordable.
* All work related injuries requiring medical treatment beyond first
aid; involving loss of consciousness, restriction of work, motion, or
transfer to another job are recordable

4.3.8. Restricted Work Cases, (RWC):


Occurs when the employee, because of the result of a job related injury or
illness is physically or mentally unable to perform all or any part of his or
her normal assignment during all or part of the workday or shift

4.3.9. Classification of Occupational Illness:

Infectious and parasitic diseases:


Malaria, food poisoning, infectious Hepatitis, Dysentery,
Lambliasis, and Legionnaires Disease.
Skin diseases and disorders: Contact Dermatitis, Allergic
Dermatitis, rash caused by primary irritants and sensitisers or
poisonous plants, oil acne, chrome ulcers, chemical burns or
inflammations.
Respiratory conditions due to dust or toxic agents: Silicosis,
Asbestosis,
Pneumoconiosis, Pneumonitis, (allergic) Bronchitis, Alveolitis,
Asthma, Pharyngitis, Rhinitis, or acute congestion due to
chemicals, dusts, gases or fumes.
Poisoning (systemic effects of toxic materials): Poisoning by
Lead, Mercury, Cadmium, or other metals; poisoning by carbon
monoxide, Hydrogen Sulphide, or other gases; poisoning by
solvents; poisoning by pesticides; poisoning by other chemicals
as such as Formaldehyde, plastics and resins.

5.0 Safety Indicator Records


Statistics are kept of all injuries, near misses, occupational diseases, accidents,
and incidents.
These statistics are based on investigative measures to ensure that the root
cause is determined and to put corrective measures in place for preventing
such an incident from re-occurring.
These statistics shall be forwarded, by Contractors and End Users, to the RLC
HSE Coordinator on a monthly basis as per the format in Attachment A.
The RLC HSE Coordinator will ensure that all the relevant data is captured in a
database as well as in the SAP HSE system.
Refer to the information gathering and reporting flow diagram. Environment
incident statistics are analysed separately from safety and health.

5.1 Record Keeping of Employee and Contractor Workers Injuries:

1. Ras Laffan Industrial shall maintain injury records for their employees
Contractors and End Users for a minimum period of five (5) years.

2. Injuries sustained by Contractors and or End Users workers within the Ras
Laffan site must be forwarded by the Contractors and or End Users
company representative to Ras Laffan HSE. These records will be kept,
for a minimum period of five (5) years, and recorded separately from Ras
Laffan Industrial City employee injury records.

3. Definite term contract personnel, temporary personnel or persons hired


and working directly under Ras Laffan Industrial Citys supervision will be
regarded as own employees and their injuries must be recorded / included
into Ras Laffan Industrial Citys employee injury statistics.

5.2 Injury Classifications

Each case is distinguished by the treatment provided i.e. if the injury was
such that medical treatment was provided or should have been provided,
it is recordable; if however only first aid was required, and it is not
recordable. However, medical treatment is only one of several criteria for
determining recordability. Regardless of treatment, if the injury involved
loss of consciousness, restriction of work or motion, or transfer to another
job, the injury is recordable.

5.2.1 Medical treatment: (MTC)


The following procedures are generally considered medical
treatment; injuries for which this type of treatment was provided

or should have been provided are almost always recordable if


the injury is work related.
Treatment of INFECTION
Application of ANTISEPTICS during second or subsequent visit to
medical personnel
Treatment of SECOND OR THIRD DEGREE BURNS(S)
Application of SUTURES (stitches)
Application of BUTTERFLY ADHESIVE DRESSING(S) or STERI
STRIP(S) in lieu of sutures
Removal of FOREIGN BODIES EMBEDDED IN EYE
Removal of FOREIGN BODIES FROM WOUND; if procedure is
COMPLICATED because of depth or embedment, size, or location
Use of PRESCRIPTION MEDICATIONS (except a single dose
administered on first visit for minor injury or discomfort)
Use of hot or cold SOAKING THERAPY during second or subsequent
visit to medical personnel
Application of hot or cold COMPRESS(ES) during second or
subsequent visit to medical personnel
CUTTING AWAY DEAD SKIN (surgical debridement)
Application of HEAT THERAPY during second or subsequent visit to
medical personnel
POSITIVE X-RAY DIAGNOSIS (fractures, broken bones, etc.)
ADMISSION TO A HOSPITAL or equivalent medical facility FOR
TREATMENT.

5.2.2 First aid treatment: (FAC)


The following procedures are generally considered first aid
treatment. For example; a one-time treatment and subsequent
observation of minor injuries should not be recorded if the work-
related injury does not involve loss of consciousness, restriction
of work or motion, or transfer to another job:

Application of ANTISEPTICS during first visit to medical personnel


Treatment of FIRST DEGREE BURN(S)
Application of BANDAGES(S) during a visit to medical personnel
Use of ELASTIC BANDAGE(S) during first visit to medical personnel
Removal of FOREIGN BODIES NOT EMBEDDED IN EYE if only
irrigation is required
Removal of FOREIGN BODIES FROM WOUND; if procedure is
UNCOMPLICATED, and is, for example, by tweezers or other simple
technique

Use of NONPRESCRIPTION MEDICATIONS AND administration of


single dose of PRESCRIPTION MEDICATION on first visit for minor injury
of discomfort
SOAKING THERAPY on initial visit to medical personnel or removal of
bandages by SOAKING
Application of hot or cold COMPRESS(ES) during first visit to medical
personnel
Application of OINTMENTS to abrasions to prevent drying or cracking
Application of HEAT THERAPY during first visit to medical personnel
NEGATIVE X-RAY DIAGNOSIS
OBSERVATION of injury during visit to medical personnel.

5.3 Recordable cases which become lost work day cases


Recordable cases which become lost work day cases due to
complications or incase of surgery at a later date will be handled as
follows:
The recorded date of the lost workday case is the date when the person
looses his first shift and not the date of the injury.

5.5 Calculation of Injury Incident Rates

5.5.1 Lost Time Injuries (LTI)

Lost Time Injuries are the sum of Fatalities, Permanent Total Disabilities,
Permanent Partial Disabilities, and Lost Workday cases.

5.5.2 Road Traffic Accident

This is an accident which involved a vehicle which has resulted in injury,


illness/damage to people, assets, the environment and companys image.

6.0 Responsibilities

6.1: Health, Safety, and Environment (HSE):


HSE is responsible for:
reporting of all related Safety Indicator statistics
Coordinating with the Managers of Contractors and End Users
Preparing the monthly report before the second working day of the new
month
Forwarding the report to concerned parties
Provide a statistical input Coordinator

6.2: Operating Contractors


Contractors are responsible for:
Assembling monthly Ssafety Indicator statistics
Accurately reporting all related Safety Indicator statistics
