ADMINISTRATIVE APPLICATION
FOR LAND REGISTRATION

With great respect, Applicant through the undersigned counsel

states, that:

(1) Applicant is CHINITA P. VIRAY, Filipino, of legal age, single,

and a bona fide resident of 035-A Happy Homes, Magsaysay
Avenue, Baguio City;

(2) Applicant is the owner of a parcel of land located in Baguio

City, more specifically described as follows, to wit:

A PARCEL OF LAND, Lot 169-B under Csd-

(af)-02-003619; containing an area of SEVEN
HUNDRED (700) SQUARE METERS, more or less;
situated at barangay Happy Homes, City of Baguio,
Island of Luzon; bounded on the North by Lot 169-A;
on the East by a road; on the South by Lot 169-C;
on the West by Lot 218 Pls 660-D.

(3) The Applicant have been in open, continuous, exclusive and

notorious possession and occupation of the same under
a bona fide claim of ownership either since time immemorial
or since June 12, 1945.

(4) The land forms part of the alienable and disposable

agricultural lands of the public domain. A certification from
the investigation reports of Bureau of Lands investigators,
showing the nature and character of the property surveyed is
hereto attached and marked as Annex A;

 (5) The original tracing cloth plan duly approved by the
Director of Lands from the Land Registration Authority and
two (2) blue print copies duly certified by the
aforementioned officer are hereto attached and respectively
marked as Annexes B and C;

(6) The advanced survey plan of the land showing the

correctness of the survey plotted in the said plan is hereto
attached and marked as ANNEX D;

(7) Three (3) copies of technical description that conforms

with LRC Circular No. 365 which is duly verified and
certified by the Regional Technical Director or his
authorized representative, is hereto attached and marked as
ANNEX E;

(8) Copy of the latest Tax Declaration or Assessment

Certificate from the Assessors Office where the land is
located, regarding the last assessment for taxation of the
property is hereto attached and marked as ANNEX F;

PRAYER
WHEREFORE, premises considered, it is most respectfully
prayed that the administrative body of the DEPARTMENT OF
ENVIRONMENT AND NATURAL RESOURCES, after hearing, to grant
the instant application and render judgment confirming the title of
herein applicant and ordering:

(a) the Land Registration Authority to issue the decree of

registration; and,

(b) for the Register of Deeds to issue the corresponding

Original Certificate of Title to herein applicant.

Other reliefs that are just and equitable under the premises are
also prayed for.
