Present Level of Academic Achievement and Functional Performance (PLAAFP)


Juaquin is a third grade student who is socially accepted by his peers. He has many friends and

loves to play sports. His mother has stated that Juaquin often gets angry and starts kicking

things when he is tired or hungry. His violent temper is out of his mothers control and she

reports that she is unsure how to help him. He will start yelling at adults and other children who

come in his path during his episodes. His episodes generally occur 3-4 times per day and will

often involve name-calling and repeatedly telling adults I dont care. Juaquin often bullies

other students in the classroom and on the playground at school and at times gets physical with

his peers.

In class, Juaquin works well independently but only completes homework 2-3 times per week. If

he is not interested in the lesson or teaching subject, he will show little effort and becomes

disruptive in the classroom. This student will often have to spend extra time with his teacher to

make up homework and class work.

Baseline for Goal 1 Juaquin will start yelling at adults and other children who come in his

path during his outbursts of temper. His episodes generally occur 3-4

times per day.

Baseline for Goal 2 Juaquin only completes homework 2-3 times per week. He will often spend

extra time with his teacher to make up homework and class work.

Annual Measurable Goals

Goal 1: Juaquin will learn to express his needs by communicating with

words in place of acting out with emotions.

Benchmarks:

1. Juaquin and the classroom teacher will work together to designate a

cool down spot when Juaquin is in need of a break by September 15.

2. Juaquin will be recognized and rewarded for a decrease in duration

and instances of emotional outburst by 25% by October 15.

3. Juaquin will be recognized and rewarded for a decrease in duration

and instances of emotional outburst by 50% by January 15.

4. Juaquin will continue to make progress towards the goal of minimal

outbursts by June 1.
BLAZO |3

Goal 2: Juaquin will increase homework completion from 2-3 times per

week to 4-5 times per week by January 5.

Benchmarks:

1. With the assistance of a homework chart, Juaquin will obtain a sticker

for each completed homework and class assignment until November

15. A small incentive will be awarded for 1 or more homework and 1

or more class assignments per week.

2. With the assistance of a homework chart, Juaquin will obtain a sticker

for each completed homework and class assignment until January 30.

A small incentive will be awarded for 2 or more homework and 2 or

more completed class assignments per week.

3. With the assistance of a homework chart, Juaquin will obtain a sticker

for each completed homework and class assignment until March 15. A

small incentive will be awarded for 3 or more homework and 3 or

more class assignments per week.

4. Juaquin will receive a small incentive for 4 or more completed

homework and 4 or more completed class assignments per week by

June 1.
