Classroom Use Policy (CUP)

Our classroom will incorporate the use of technology to further advocate student
learning as well as entertainment. There are a limited number of computers and tablets
available in the classroom, so it is important that we use this technology in an appropriate
and responsible way. Our Classroom Use Policy (CUP) ensures that students and parents
understand the rules set by Mr. Eversole as well as rules set by the school for the use of
technology in the classroom.

Classroom Rules:
1. Students will treat teachers, classroom, classmates, and personal property and
space with respect.

2. Students must raise their hand before talking if they have a question or would like
to say something out loud.

3. Students will come to class prepared and ready to learn.

Computer/Tablet Rules:
1. Students will log in/out each time they use a computer/tablet. Students will be
allowed 20 minutes for computer/tablet time.

2. No food or drink allowed at or near computer/tablets.

3. The computers/tablet will be used only for educational content. Students are only
allowed on pre-approved websites, apps, or educational games. Any additional use
of the computers/tablet must be permitted by Mr Eversole.

** Failure to cooperate with basic use of the computers/tablet will result in the following:

1st offense: Verbal warning

2nd offense: Removal from computer/tablet for the remainder of the day
 3rd offense: Removal from computer for the remainder of the month along with a
letter sent home to parent/guardian.

4th offense: Referral to Principal with disciplinary actions including suspension

from computer. Additional parent/guardian conferences may be held.

Code of Conduct Agreement

Upon reading the Classroom Use Policy (CUP), Mr. Eversole requests that both the
student and parent/guardian sign in the provided spaces to indicate that he/she
understands the rules and consequences regarding technology use in our classroom.

As a student at Henry Elementary, I hereby understand and agree to the rules and
behavior of the Classroom Use Policy when using the computers/tablet not only in Mr.
Eversoles classroom, but for all computers and tablets in Henry Elementary. I understand
that if I do not adhere to these rules and behaviors, disciplinary actions will be taken. My
computer/tablet use is a privilege and I will respect all equipment and follow all
technology/classroom rules .
