APPOINTMENT LETTER

We are pleased to engage your service as FOOD ATTENDANT on the following terms and
conditions.
a.) Your service is commencing on NOVEMBER 23, 2016 to DECEMBER 31, 2016
Co-terminus with our Service Agreement at CROWNE PLAZA MANILA GALLERIA.
b.) The engagement of your service with this company is as Project Employee. You shall be entitled to
the rights and privileges same of a regular employee which includes pro-rata 13 th month pay, 5 days
incentive pay, rest days, overtime pay, holiday pay, Social Security, Philhealth, Pag-ibig and welfare
benefits.
c.) You shall receive the amount of P 481.00/DAILY Effective JUNE 2, 2016
d.) Your duties and responsibilities are to be explained by the clients supervisor.
e.) In the event of no scheduled duty or assignment, the policy of no work n, no pay applies.
f.) You shall abide by the rules and regulations of the company and those of your client where you are
assigned.
g.) In order to maintain our standard of service, Primepower reserve the right to conduct random
performance evaluation to all staff deployed to our client with in this service period.
h.) All necessary requirements must be submitted before undergoing an actual training (OJT).
Incomplete requirements will not be endorsed for training.
i.) In the event that the client requires your substitution or replacement by reason of negligence,
discourtesy, or personality, or due to your unsatisfactory performance or behavior, or simply
expiration of this contract/project, the agreement of your services with the company shall
automatically be terminated.
j.) Upon expiration of your contract, and if in any case this will be extended, you will be issued another
contract based on the evaluation & recommendation by your assigned client.
k.) You will surrender your ID (Identification Card) when requesting for certification and clearances for
verification purposes.
l.) You cannot be directly hired or absorbed by another agency within the same company you are
assigned. Likewise, you are not authorized to resign for reason that the client directly hires you unless
otherwise permitted. Or it would result to a breach of contract where you may be held liable for
consequential damages thereof.
m.) You shall be held liable for any damages which may occur due to your negligence during tour of duty.
n.) In the event that you have violated any of our work performances while on duty particularly on cases
such as theft of property or cash amount, either allege or not and investigation showed of your
admittance to the incident, you are required to pay to Primepower the total cost of the charges of the
loss amount and or property. Failure to do so and in the absence of a case filed by the main victim,
Primepower Manpower Services shall have the right to file a case against you with the proper
authorities.
o.) You shall not be allowed to enter the Clients premises, either as Clients Guest or Visitor, during the
first Six (6) months from the date of your contract expiration.
p.) Resignation should be filed Thirty (30) days before effectivity to enable to
turn over all of respective responsibilities. Failure on your part will put all benefits on hold.
q.) Separation/resigned from the company with unfinished contract should have approved clearance from
client and the agency for release of all claims.
r.) This appointment letter shall be subject to and executed by this company with the client.
