REQUIREMENTS
Application letter, addressed to the Director, Mines and Geosciences Bureau-Central Office, Attention: MGB
Local Scholarship Committee.
MGB Scholarship Application Form
Two (2) passport size photos
A copy of latest Income Tax Return (ITR) of parents or guardians. In case the ITR is not applicable, a
certification from the BIR as non-registered taxpayer is required.
Photocopy of Form 138 (with final grades)
Principals Certification as candidate for graduation belonging to the top 10% of the total graduating
students
Certificate of Admission (CA) from school/university/college where he/she intends to enroll. In case the CA
is not yet available, a certified copy of the application for Admission shall be temporarily accepted.
Medical clearance as certified by a government physician
Certificate of good moral character as certified by the principal
Police or NBI Clearance

Transcript of scholastic records
Certificate of Admission (CA) from the school/university/college where he/she intends to enroll. In case the
CA is not yet available, a certified copy of the application for Admission shall be temporarily accepted.
Certificate that he/she is currently enrolled in the school/university/college taking up either BS Geology,
Mining Engg or Metallurgical Engg course as certified by the school Dean or Department Head
Certificate that he/she is currently enrolled in the school/university/college and has been granted
permission to shift either to BS Geology, Mining Engg or Metallurgical Engg course as certified by the
school Dean or Department Head
Medical clearance as certified by a government physician
Certificate of good moral character as certified by the school Dean or Department Head
Police or NBI Clearance
