Please see below the accomplishment that the undersigned had undertaken during his Field
Deployment in CSWDO- Field Operation Division at Barangay Batomelong.
Attended and observed weekly directional.
Assisted and interviewed walk in clients.
Assisted and .observed in the conducted FRAME orientation.
Attended orientation regarding CLEAN Gensan Family Card.
Conducted survey regarding Clean Gensan Family Card.
Assisted the Senior Citizens during the released of Social Pension.
Intake/ interviewed to clients who can avail CLEAN Gensan Family Card.
Made an LGU and Clean Gensan assessment.
Observed during counseling CAR, CICL.
Encode Frame documents. /filed intake forms.
Conducted home visitation.
Prepared and submitted Weekly Accomplishment.
Prepared Social Case Study Report.

Below are the insights that the undersigned given during the deployment:
Always focus during client interviewing
Be active in times of assisting clients.
Carefully reviewed the receive documents.
Learned in the newly updated government programs.
Learned on drafting Social Case Study Report.
Not all clients are telling the truth.

Please take note that undersigned had surpassed the following challenges.
Overcome my shame in socializing people.
Control my emotion in times when the clients is too emotional.
Enhance my skills in taking computer works.
Uneasy to construct the right grammar.
Challenged in conducting home visitation.
Be patient always.
Below are the recommendation on the Future Deployment of Field Work Student
To the next batch of On-The-Job (OJT), we recommended that they must boost
their capabilities as a Social Work Students in learning new structure codes and
experiences to the real job. Do not limit by working alone, instead gather all ideas,
even if its hard dont lose hope. Always remember that every problem has a
solution. Work as a group with willingness whatever the task that we encountered.

Dear Sir / Madam:

I would like to convey my gratefulness to you and your colleagues for the excellent OJT
which I have undergone in your institution. The skills and knowledge which I have gained
throughout my practical training I perceive as very valuable component in my future career
development. As you know this OJT was part of my educational curricula and therefore I have to
thank you for providing me this opportunity.

I would especially like to thank our AFI Mrs. Ronnavel S. Temblor, and to our Area Worker Mrs.
Didi Q. Necor for their generous support, coaching and companionship during my OJT. I shared
their expertise with me very generously and I have learned a lot from them. My thanks and
appreciation also to our Division Chief Mrs. Arabecque TW. Batilong for her willingness to help
us, and above all to your good companionship.
