dentist for a dozen years, Dr. Isadoros Mereos, whose office is four blocks from
yourhome,hastoldyoupreviouslythatyouneedarootcanalandcrownforthat
tooth,whichhewillbegladtodofor$2,700.
Now you are an elderly man who is having difficulty getting around because of
paininthechestandfatigue,conditionsforwhichyouhavevisitedyourdoctor.
Yourincomeisbelowthepovertylevel,andyouhavearound$300monthtolive
onafterrentandutilities.
Noproblem,youtellyourselfafteryoutakeseveralpainkillers.Youhavedental
insurance,orsoyouthink.
Whatyouactuallyhaveisaninsurancecardindicatingthatyouaresupposedto
have dental insurance with United Health Cares Preferred Care Partners.
Whetheryoureallyhaveinsuranceremainstobeseen.
Unfortunately,yourdentistdoesnotparticipateinthatMedicareAdvantageplan,
and he would not say why. You need to know who does participate in your
neighborhood.Yourecallthatwhenyousignedupfortheplanwayback,several
dentistswithinwalkingdistanceparticipated,andthatadvertisementwasoneof
themainreasonsyoubecameamember.
On Saturday, May 6, you call the Customer Service number on your insurance
cardforDental(8553516163),andgetahighpitchedbuzzingsound.Apparently
itisafaxnumber,althoughitisnotlistedonthecardassuch.Thenyoucallthe
mainCustomerServicenumber,forPreferredCarePartners(8662317291).
After going through several steps with the automated answer center, including
identifying your issue as dental pain, and being warned that your call may be
recorded,thedronetellsyouthatifyouhaveanemergencytocall911because
youhavecalledoutsidenormalworkinghours,inyourcase,theweekend.Forget
aboutit:theFireDepartmentwillnotsendanambulanceoverfordentalpain.
You are fortunate to have the cell phone that a sympathetic vice president of
Sprint,StevenM.Shoecraft,gaveyouforaccesstotheInternetafterthephone
youhadfortelephonecallsandtextwassuddenlydeclaredobsolete.
You take two more pain killers and use your new fangled Iphone to go to
www.myPreferredCare.comlistedonyourPreferredCareinsurancecard.

page,youneedtosignin,buttheinformationonyourcarddoesnotwork,and

Hello, I have a painful dental emergency, and I have been unable to obtain a
scheduleofprovidersandascheduleofbenefitsalthoughPreferredHealthCare
Partners is supposed to provide dental insurance via Solstice and my premiums
arepaidthroughtheSSA.SolsticewebsiterefersmetoPreferredCare,Preferred
CaretoSolstice.TheWebsiteishorrible.Nosearchfacility.Icannotsignin.Also,
the Preferred Care website advises to register prior to logging in, but does not,
like any other website, provide instructions or a facility for registration on that
page.Iamsurprisedwiththeinadequaciesofthewebsite,andIhopethatisnot
indicativeofthecompanyasawhole,whichhasstatedonformssenttomethat
mydoctor,whoislistedinthehardcopycatalogue,isnotaparticipatingprovider,
andafterahalfhour'sconversationwitharepresentative,hasnotcorrectedthat
issue nor another issue, involving the mishandling of an $8 drug bill, which is
beingkickedaroundtotwootherinsurancecompanies,CAREPLUSandHUMANA,
bothofwhichpaidthebillalthoughIamnotamember,andaredunningmefor
it.Ihopethemanagedhealthcaresystemisnotsomismanaged.Norecognition
of member number on cards, My Preferred Care No. is XXXXX154800, and my
SolsticeNo.isXXXXX154800.PreferredChoiceDadePleaserespondimmediately.

is denied because you, the victim, are routinely blamed for the insurance
companys mistakes, on 21 July you will be respectfully told by Myrna James,

Ihavemadeseveralattemptstocontactyouviaphonetothenumberwehave
onfile(3054385000)butitstateslinebusyanddisconnectsthecallwithoutan
optiontoleaveavoicemessage.
That is no wonder, because 3054385000 is not your number. It is the listed

numberofPreferredCarePartnersMiami.Moliereshouldbewritingupacomedy
oferrors!
Finally the weekend is over. You recall from an old listing that a highly
recommended dentist, Dr. Amini Bahman, with offices at luxurious 119
Washington Avenue, participates in your socalled dental insurance plan. It is
Monday,youareinalmostunbearablepain,butyougrityourteethandcallthat
number. His administrative assistant says she is not aware of Dr. Bahmans
participationinthatplan.
NowyoufinallyreceivefromSolsticerepresentativeHernandoJimenezaPDFof
providers.Thereisonlyonelistedinareacode33139,andthatdentistisnotonly
unavailableatthetime,butisoneyoudonottrustbecauseofaveryhighquote
given several years prior in comparison to a quote given by Dr. Mereos for the
samework.
No, Dr. Bahman is not listed, the painkillers are not working, so you call Dr.
Mereos,andhewilltakeyouimmediately,andhewilldiscounttheworkdownto
$2,200pluslab,sothereyougo,andyoushellouthalf,$1,100,fortherootcanal.
Thepainisgone.Youdonothaveanother$1,100forthecrown,andwithouta
crown you are told the tooth is at risk. The reason you did not have it pulled is
becauseitisoneoftwoteeththatyouhaveleftonthelowerjaw,soyouneedit
to anchor the denture. If it is lost, you will need a new lower denture, and you
maybemiserablefortherestofyourlife.
Well, you did what you had to do. And it was not easy walking even that short
distance,toDr.Mereosoffice,inpain,yourheartpounding,andfeelinglikeyou
arewalkinginquicksand.
ToobadDr.Bahmanwasnotonthelistofprovidershesuppliedforarea33139,
since$1,100wouldhavecoveredoutofpocketforboththerootcanalandcrown.
HernandoJimenezexplainsthatdentistscomeandgofromthenetwork.
GuesswhathappenedafteryouweretreatedbyDr.Mereos?Right,Dr.Bahmans
officecallsyoutoinformyouthatheisonthelist.AndHernandoJimenezsends
youanotherlistshowingthatthereareseveraldentistsparticipatinginareacode
33139,includingDr.Bahman,theverydentistyoumentionedtohimasnotbeing
onthefirstlisthesent.
Youarerightfullyangry,andwhipoffamessagetoHernandoJimenezthathehas
costyou$1,100andagreatdealofsuffering.Hedoesnotapologize.Infact,he
blamesthewholethingonyou.
Yousee,hesays,thereisabiglistandalittlelist,andthebiglistdoesnotinclude
the dentistsonthe littlelistthatareinarea33139i.e. MiamiBeach,yourarea,
nottomentionseveralclosetoyourhome,includingDr.Bahman,soyoushould
havegonetoanyoneofthedentistsonthebig,15mileradiuslist.
Of course that standard strategy, of blaming the victim for getting beat up, and
notevenapologizing,givesyoucausetoresolvetopursuetheinsurancecompany
totheendofyourdaysuntiljusticeisdoneornot.
Of course Myrna James in the grievance department is moved by the standard

strategytocolludewithHernandoJimenezinthedenialofresponsibilityandthe
casting of blame on you for the error. After all, this is the corporate world, and
they have a program from which they must not vary, one that may save them
fromfurtherdeficits.Inshort,youmightaswellbetalkingtorobots.
Havefaith,however,foranapparentlyhumaneandreasonablerayisemittedby
Myrna James on 21 July, in the same email she advised of her many efforts to
contact me by calling the wrong number, which did not answer, the number of
UnitedHealthCaresPreferredCarePartners.
Please understand, she wrote, we are in the business of helping all of our
membersasbestwekind(sic).Atthistime,wedonothaveanyclaimonfile.Idid
contact the office of Dr. Mereos and requested they submit a claim for the
servicesyoureceived.TheyoungladyIspoketoJudystatedshewillgetthe
claiminonMonday.Shedidnotexplainthereasonforthedelay.
Noticethekind,whichIinterpretedasasignofgoodintentionswhetherornot
it was a Freudian slip. We are kind to kin and others health care cares for. In
anycase,thehumaneintentionofthemessagewasclear.Yes,aclaimwasbeing
solicited from a dentist whom she knew was not a provider, a fact that had
already led to a blank denial of my claim. So here was a responsible company
willingtocorrectanerror.Infact,suchacorrectionwouldcostitnomorethanif
Dr.Mereoswasintheirnetwork.
Igladlyrespondedon21Julywith:IshallstandbyfortheresponsefromJudy.I
paid$1,100fortherootcanalataspecialdiscountrate.TheproblemisthatifIdo
not have Dr. Mereos do the crown right away and refit the denture, I shall lose
the tooth. I was indeed worried about the effort to hold me responsible for an
error that should have been admitted, and by no response to my request for
records, so I proceeded with research into responsibility records in the Miami
Dade Countybidandfilings inseveralstates.Needlessto say,Iamgladtohear
fromyoutoallaymyconcernsfornow.ThewayIseeit,ifSolsticetakescareof
the crown work, every party will be no worse off than if the matter had gone
throughthenormalnetworkprocedure.David
Indeed, I was become quite suspicious of Solstice, which provides insurance in

several states under that brand name, and had included its president and
beneficialownerMichaelFlax,aprominentdentist,withacopyofmygrievance.
I checked with Judy at Dr. Mereos office to discover that she had in fact
submittedaclaimtoSolsticeasrequestedalongwithXrays.
AndthenMyrnaJamesweighedin,reversingherselfon31Julywiththiscasuistry:
The request for the claim from the office of Dr. Mereos is only so we could
process the claim and provide you with a formal document which explains the
processingofyourclaim.However,Dr.Mereosisnotrequiredtosubmittheclaim
asheisOutofNetwork.Icalledhisofficeonyourbehalfasacourtesy.Ifyoufelt
that by requesting the claim we were considering in approving your claim, I
apologize. That was never my intention. Once the claim is received and
processed,itwilldenyforservicesrenderedbyanOutofNetworkProvider.
All that and more from an insurance company that advertises itself as receiving
rewards as the BEST around. That may very well be true, and is indicative of
what is wrong with not only health insurance but government in this country:
mismanagement. Managed health care was supposed to provide better health
care and save money to boot, but mismanaged managed health care is costing
livesandtreasureandjackinguppricesforeveryitemonthebenefitschedules.
The pain and emotion distress I have suffered as a result of my membership in

Solsticeisnotevenadropinthebucketcomparedtothatsufferedbyhundreds
ofthousandsofpeopleintheUnitedStates.
Theelderlyareespeciallyhelpless.Iaskedyoutoimaginethatyouwereme,and
to consider what you would do if confronted in my situation with my little
problem. You probably would wind up frustrated by the system and wind up
cheated of your premiums and health care if you were my age and had my
income. Wherefore I speak for you, in my little way, despite the fact that the
toothisstartingtogivemepainagainforlackofacrown.

Madame Myrna James: With all due respect, you are not listening, or are
exhibiting the denial and blaming mode, in collusion with the Solstice
representative,whoprovided mewith misinformationaboutnetworkproviders.
That representative initially provided me with a list of providers that did not
include a preferred provider four blocks from my house, and listed only one
providerin33139,whowasnotavailablefortheemergency.Theadminforthat
preferredprovider,whoreceivedacopyofthatlistfromme,didnotknowwhy
the provider was dropped from the list. My regular dentist, Dr. Mereos, who is
not in the network, however, was ready to take me right away. I went for the
immediately relief, and on the same day the admin for the preferred provider
called me to say that she had done some checking, and yes, come over for the
emergency because that group was in the network. Too late I was out $1,100
out of pocket for the root canal. I told your representative that he had cost me
$1,100withthenewlistandhisrhetoricabout howdentistscomeandgofrom
thenetwork,andthatafterhehadthenameofthepreferrednetworkprovider.
Oh, no apology came from him, just here is the right list, and it is all your fault
becauseyouareresponsible.Wrong.Well,soyousaidyoucontactedDr.Mereos
for a claim, leading me to believe that you are therefore going to waive the
networkrequirement,Iwaitedaweek,withwhatnowappearstobeafalsesense
ofsecurity,afteradvisingyouthatImaylosethetooth,crucialtomyholdingmy
denture, if I do not get the crown right away, and now this from you. This
behavior in my opinion is very unprofessional and incompetent, unlike what I

Providers sent is within a 15 mile radius which is a reasonable and customary
distance to seek treatment. Again, it was your decision to go to an Out of
Network Provider even after providing you a list of InNetwork Providers. The
second list provided was a narrowed list including only dentists within a 5mile

We used the zip code we have on file for you and as I explained before, the

network providers in my Zip Code, particularly one that I contacted in an

disconnects the call without an option to leave a voice message. Please

At this time, we do not have any claim on file. I did contact the office of Dr.

our default parameters of 15 mile radius, the fact is we provided a list of seven
pages of in network providers within 15 miles of your zip code which is a
reasonable distance to seek innetwork services. If you require any additional

suitable implant; A dentist that was on an old network provider list was

provider,andtheadmindidnotknowabouttheinsurancebutpromisedtocheck
on it, wherefore the pain was such that I contacted Dr. Mereos, my preferred
dentist for many years, and I went in for the emergency root canal, which
immediatelyrelievedthepain,atwhichjuncturetheadmincalledmetosaythat
groupwasindeedonthelist,andIreceivedanotherlistfromyourrepresentative,
and indeed, that recommended provider was not only on that list, but a close
examinationrevealsthat33139iscrawlingwithnetworkdentists,atwhichpointI
toldtherepresentativehehadjustcostme$1,100,andhewentintodenialmode
andshiftedresponsibilityontome.Bytheway,anexaminationoftheemailrefers
toavailabilityofanonlinesearchfacility,butIfiguredthefirstlisthesentmewas
authoritative,and,infact,therewasviaPreferredHealthCarePartnersanonline
dental provider search facility that weekend, but it would not take an inquiry
without a password. Further, note that a 33139 search of the list the
representative sent me does have, as you noticed, a single dentist, who was
unavailableatthetime,andwhomIonanotheroccasionsatforanexamanddid
not care for at all. It did not occur to me to search on "Miami Beach", which
returns other information on groups with addresses in Miami, and members of
those groups may have dentists visiting somewhere on Miami Beach other than
33139orevenin33139.Anyway,theideathatacountywidesearchnotincluding
dentists within walking distance from me and excluding those that were is
confounding.Incidentally,thesecureSolsticefacilitydidnotallowthismessageto
be sent VIA REPLY ALL, that is, to all supposedly secure addressees in Solstice
email. But it did allow a REPLY only to Appeals. Can you imagine how many
seniorshavethepatience,accessto,andfamiliaritytopursuegrievances?Allin
all,IhavealwayshaddifficultyunderstandingwhyweshouldhaveanHMO,and
why not a choice of dentists, which is of course presumably justified by better
