AFFIDAVIT OF DISCREPANCY

I, EVELYN J. BANTEGUI, Filipina, of legal age, married and presently residing at #448
North 9, Angela Village, Talon 4, Las Pias City, after having been duly sworn to in
accordance with law, hereby depose and state;

That I am the wife of JOHN BOLIVER BANTEGUI who was born in June 24, 1951 at
Barangay Sto. Nio, Roxas Capiz City;

That the birth certification of my husband duly filed and registered with the Office of
City Civil Registrar, Roxas, Capiz City;

However, in our marriage contract his name appeared therein as JOHN DAVID B.
BANTEGUI instead of JOHN BOLIVER BANTEGUI;

And in the certificate of live birth of his son, RODEL JACINTO BANTEGUI, his name
was recorded as JON B. BANTEGUI instead of JOHN B. BANTEGUI;

Likewise, in the certificate of live birth of her daughter SHEELA JHEAN JACINTO
BANTEGUI, his name was recorded as JOHNNY B. BANTEGUI instead of JOHN BOLIVER
BANTEGUI;

The foregoing discrepancy is merely incidental and very insignificant and does not
digress from the fact that the name JOHN B. BANTEGUI, JOHN DAVID B. BANTEGUI, JON B
BANTEGUI AND JOHNNY BOLIVER BANTEGUI, all refer to one and the same person, for all
its legal intents, effects, consequences and purposes;

That although said childbirth record is duly filed and registered with the National
Statistic Office and in his passport his true and correct name is JOHN BOLIVER BANTEGUI;

That I have executed this affidavit for the purpose of establishing the veracity of all
the foregoing information and hereby declares under oath that names JOHN B . BANTEGUI,
JOHN DAVID B. BANTEGUI, JON B. BANTEGUI AND JOHNNY B. BANTEGUI refer to only but
one and the same person;

I am executing this affidavit to attest to the truth of all foregoing facts and for its
legal intents, effects, consequences and purposes.
