APPLICATION
The undersigned, Samantha Gaile A. Reyes, hereby applies to have
the land hereinafter described brought under the operation of the Property
Registration Decree, and to have the title thereto registered and confirmed:
AND DECLARE:
1. That the applicant is the owner of the land by virtue of possession
in accordance with Section 14 of said Decree together with the
building and improvements thereon.

2. That said land at the last assessment for taxation was assessed at
P 250,500.00, Philippine currency, and the buildings and other
improvements at P 300,000.00, Philippine currency.

3. That to the best of my knowledge and belief, there is no

mortgage or encumbrance of any kind whatsoever affecting said
land, nor any other person having any interest therein, legal or
equitable, or in possession.

4. That the applicant has acquired said land through her

predecessors-in-interest who have been in open, continuous,
exclusive and notorious possession and occupation under a bona
fide claim of ownership since 1935, as evidenced by Tax
Declaration No : E- 006-03066 (hereby attached as Annex A)

5. That said land is currently occupied by the undersigned and her

parents namely, Norisa Abergas and Faustino Reyes.
 6. That the names in full and addresses, as far as known to the
undersigned, of the owners of all adjoining properties, and of the
persons shown on the plan as claimants, are as follows:

7. That the applicant is 22 years of age, single, Filipino and a resident

of Lot no. 1008, Pls-209-d-13, Klinan 5, Mabuhay, General Santos
City.

8. That the following documents are attached hereto and made a
