Introduccin
El TEP es una condicin grave, muchas veces fatal, que ocasionalmente
requiere un tratamiento invasivo. Dalen calcul que el 10% de estos pacientes fallecen en
forma sbita, antes de ser asistidos1. El 70 % del grupo restante evolucionaba segn este autor
sin diagnstico, con una mortalidad estimada en 30 %. El tercio restante, conformado por los
pacientes diagnosticados y tratados oportunamente, mostraba baja tasa de recurrencia y una
mortalidad entre 2-8%, con excepcin de aquellos con compromiso hemodinmico de inicio, en
quienes la mortalidad alcanza 20 a 30 %.
A pesar de los buenos resultados publicados por sus creadores 5,6, el uso de
este dispositivo no se generaliz. Las razones fundamentales de la poca aceptacin son la
necesidad de una venotoma o de introductores venosos de gran calibre (16-24 F), la poca
maniobrabilidad del sistema y la necesidad de realizar muchos pasajes para aspirar
completamente el trombo. Adems, los resultados obtenidos en otros centros no fueron
similares a los originales. 1,7.
En el mismo sentido, algunos grupos intervencionistas japoneses han
comunicado resultados alentadores con la aspiracin manual del trombo utilizando catteres
gua de angioplastia por su amplio lumen (8 F) 8,19.
Indicaciones y Complicaciones
La ausencia de series grandes de pacientes que confirmen los resultados
reportados y de estudios comparativos entre los distintos mtodos, son limitaciones para
establecer las indicaciones precisas del tratamiento percutneo.
