NOTIFICATION OF WORKPLACE ACCIDENT

It is with incredible sadness that I inform you of the grave loss of a valued City
employee.

Last night at 6:00 PM, dispatch received a call of a workplace accident at the Citys
Waste Water Treatment Plant located at 700 W.T. Field Drive in the City. Police, Fire
and Guilfoyle ambulance responded and found an injured City employee. The
employee was transported to Samaritan Medical Center where he was later pronounced
deceased.

Today, Police Detectives and Waste Water Treatment staff are meeting with Public
Employee Safety and Health (PESH) inspectors, who will assist in the accident
investigation.

Additional information will be shared once the employees family is notified and as
further information of the incident is made available.

On behalf of the City of Watertown staff, our heartfelt condolences and prayers are sent
to the family of this employee.
