INSTRUCTIONS FOR USING THE STAFF FLIP CHART

Familiarize yourself with the Flip Chart; your life and/or the lives of others may depend upon it
Keep the Flip Chart readily accessible for use. Maintain the Flip Chart near emergency evacuation maps
Refer to important telephone numbers listed inside the front jackets of the Flip Chart to report emergencies and for follow-up referrals/
actions
Secure the Flip Chart from theft. Report the loss of a Flip Chart immediately to administration
No emergency procedures are inclusive for every incident. Be prepared to use the Flip Chart with compliance as determined by
assessment of the incident and individual emergency training
You can gain valuable insight to emergency management by becoming familiar with procedures outlined in the Flip Chart in conjunction
with practice of regular emergency response drills; Fire exit drills, Disaster exit drills, Shelter In Place for Inclement Weather and/or
Hazardous Chemical emergencies and/or Lockdown for Intruder
Use the Flip Chart as a part of ongoing site-based emergency management. Update critical leadership/role information, e.g., site-based
Incident Command System, telephone numbers, safety committee members, CPR/First Aid/AED trained employees, etc., as needed
Report to administration anyone who shows an unusual interest in site-based emergency procedures

If your campus receives a FIRE MARSHAL INSPECTION REPORT or a FIRE CODE CITATION following an inspection, please fax the document(s) to
CFS, CUSTOMER CARE CENTER, 713-556-9275, or scan documents to send by email to: firecodeviolations@houstonisd.org. Do not delay this action!
When a fire safety infraction is identified by a Fire Marshal, the school is issued a NOTICE OF VIOLATION. HISD has 30 days to correct the infraction.
Failure to correct the infraction in the allotted time may escalate action to include a Re-Inspection Notice and fee. Once the Re-Inspection Notice is
issued, HISD has 10 -15 days to correct an infraction. Failure to correct an infraction may result in a CITATION being issued and/or repeated CITATION.
A CITATION will result in HISD having to appear/respond in court to a fire safety infraction with probable assessment of penalties and fines.
Individual campuses are responsible for correcting violations such as inappropriate use of extension cords, improper storage, and failure to
post emergency plans/exits with designated fire extinguishers and AEDs.
