Traveler will need to print, fill out, and have completed application Contact Person

1. Submit Arizona Statement of Citizenship and Alien Status for Telephone: (602)-297-0276

2. Request final official transcript with date of graduation and degree earned be mailed directly to the Arizona State Board of
Physical Therapy by the college or university. If you have not yet received your degree, a letter of Candidate for graduation
on school letterhead signed by the program director of your school is required stating that you are a candidate for a degree as
a physical therapist/physical therapy assistant at the next scheduled graduation date, or have completed all the program
requirements and the date of graduation. Please note that licensure will not be issued without the final transcript.
3. Request NPTE score be sent directly to the Arizona Board from the FSBPT. If you have not taken the NPTE you must
register, pay for, and take the exam through the FSBPT
4. Request verification of licensure or certification from every state in which you hold or have ever held a license or certificate
in physical therapy. Verification must be sent directly to the board from the state licensing jurisdiction.
5. Register to take the AZLAW (Jurisprudence) exam at www.fsbpt.org. You must be cleared by the Arizona Board to take the
exam. To be cleared for the exam, the license application and all additional required documentation and fees must be received
by the board. Once cleared, an email will be sent to you indicating you can schedule to take the exam.
6. Obtain and provide a valid Arizona Fingerprint Clearance Card by the Department of Public Safety to the Arizona Board.
You can apply for the card online at http://fieldprintarizona.com. You can also contact them at 602-223-2279 or fax a request
to 602-223-2947. In addition to the fingerprint card, you must fill out the Temporary Addendum Fingerprint Clearance Card
Application available on the board’s website. Once your fingerprints have been obtained mail the card and addendum form to
the board with your application.

License Fee: For PT’s a $260.00 application fee is due for an application submitted between September 1 of an even numbered year
and August 31 of the following odd numbered year. A $190.00 application fee is due for an application submitted after September 1 of
odd numbered year to August 31 of the following even numbered year. Fee must be paid by money order or cashier’s check, make
payable to Arizona State Board of Physical Therapy.

For PTA’s a $160.00 application fee is due for an application submitted between September 1 of an even numbered year and August
31 of the following odd numbered year. A $120.00 application fee is due for an application submitted after September 1 of odd
numbered year to August 31 of the following even numbered year. Fee must be paid by money order or cashier’s check, make payable
to Arizona State Board of Physical Therapy.

License Time: Licenses must be approved by the board and can only be done at board meetings. The board meets on the 4 th Tuesday
of every month so documentation needs to be submitted in full for them to review your application at that time. One should plan 3-4
weeks for their license to be granted if all documentation was submitted correctly and their fingerprinting card has been received.

License Duration: License is valid until August 31 of even numbered years. Full duration of license is two years if applied for before
the deadline of September 1 of even numbered years.

License Renewal: To renew your license, a renewal application must be submitted before September 1 of even numbered years. If
your application is received after this date, your license will have to be reinstated rather than renewed. PT renewal fee is $260.00 PTA
renewal fee is $105.00. Fee must be paid by money order or cashier’s check, make payable to Arizona State Board of Physical
Therapy. PT’s must complete 20 hours of continuing competency courses during each two year licensure period.
