JOINT-AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, EDNA D. PARRILLA and MANSUETA A. TAGALOG, both of legal age, Filipino citizens
and residents of Barangay Cogon and San Isidro, Ormoc City, Leyte, Philippines, respectively,
after having been sworn in accordance with the law, thereby depose and say:
1. That we personally know Ramonito Delgado Marquez and Yulanda Malinao Corpus,
residents of Barangay Manlilinao, Ormoc City, Leyte;
2. That they begot a child named RAMONITO CORPUS MARQUEZ, JR. who was born
on June 9,1979 at Ormoc City;
3. That the fact of birth of the above named child was not registered in the office of the
Local Civil Registrar of Ormoc City due to negligence and unawareness of the Civil
Registry Laws, Rules and Regulations;;
4. That we know the above acts for we are neighbors and friends for a long time and up
to the present ;
5. That we are executing this affidavit in order to attest the fact and truthfulness of the
foregoing statements for the late registration of birth of RAMONITO CORPUS
MARQUEZ, JR., and for whatever legal purpose this affidavit may serve;
6. That the same is not made for the purpose of fraud or neither to prejudice of the
government or any third person but in attestation to the facts we have hereunto
stated.
