1. Accomplished Entrance Application Form (EAF) - Senior High School
2. Photocopy of J.H.S. Report Card
3. Receipt of Payment (Entrance Fee)

B. Fee:
1. Entrance Examinations – PHP200.00
2. Pre-Registration – PHP500.00 (Non-refundable but will be credited to the down payment upon enrollment.)

C. Pre-Enrollment Procedure

1. Accomplish Entrance Application Form (EAF) - Senior High School for examination
2. Submit accomplished Entrance Application Form (EAF) - Senior High School, Photocopy of J.H.S. Report Card to the Admissions,
Scholarships and Placement Office. (ASPO)
3. Secure payment slip from ASPO.
4. Pay entrance examination / pre-registration and get receipt of payment at the USA Accounting Department/Cashier
5. Present payment receipt to the ASPO and get the entrance test schedule.
6. Take Entrance Examination as scheduled at the Guidance Services and Testing Center (GSTC).
7. Get entrance test result from the ASPO.
8. Get Interview schedule from the Senior High School Office.
9. Submit oneself for interview.
