Improper use 2 2 4 Operatives shall always select the correctly designed tool 2 1 2
for the job.
Prior to use the Operative shall inspect the hand tool to
ensure it is safe to use.
Operatives shall seek instruction / training if unsure how to
use the hand-tool.
All tools are maintained to ensure they are safe to use.
Any damaged tools shall be reported to the Site
Supervisor prior to being taken out of service for repair.
Operatives shall not misuse hand tools ie. Screwdrivers as
chisels,
Spanners as hammers.

Dropping hand-tools 3 2 6 All operatives to use a restraint lanyard on all hand tools 3 1 3
to prevent tools falling from height.
Tools to be returned to the stores or tool vaults when not
in use.

Damaged tools 2 2 4 All tools are maintained to ensure they are safe to use. 2 1 2
All damaged tools shall be reported to the site supervisor
prior to being taken out of service for repair.

Severity (S): 1-minor 1st aid, 2-Minor hospital referral, 3-RIDDOR (over 3 day), 4-Major injury, 5-fatality
Likelihood (L): 0 - Very Remote, 1 - Remote, 2 - Occasional, 3- Probable, 4 – Frequent
Hazard Rating S x L (0-3 = Low, work can 4-7 = Medium, Work needs additional controls, 8 or more = High, Work shall not commence)
