Dear Ms. Narsolis:

Congratulations! You qualified for the 2015 RA 7687 Bachelor of Science

Scholarships out of the 42,987 applicants who took the national examination last
September 21, 2014.
You are to pursue a 4- or 5-year Bachelor of Science or Engineering course in any of
the DOST-SEI priority fields of study at a state college/university or an institution identified
as a CHED Center of Excellence/Center of Development (COE/COD). Make sure that you
seek admission at the University you intend to enroll in. Please refer to the attached list of
priority courses and list of CHED COE/COD.

You will be advised to report with one of your parents or legal guardian for a
scholarship orientation and signing of the Scholarship Agreement as soon as we
receive the printed copies of the agreement.

Please bring the following requirements during the orientation:

1) 2015 Community Tax Certificate or current passport of Parent/Guardian

2) P100.00 for the notarization fee of the Scholarship Agreement
3) One (1) copy of 2x2 and two(2) copies of1x1 latest photo
4) Notarized affidavit of guardianship or special power of attorney (for legal
guardian)

Upon receipt of this notice, please inform us immediately whether you are accepting,
deferring or not accepting the scholarship. You may contact the undersigned at (065) 212-

Please fill-out the enclosed Reply Slip and return it not later than 31 March 2015.

Moreover, if we do not hear from you after 30 April 2015, we shall consider this
as no interest on your part to be an S&T scholar and shall serve as basis for the
forfeiture of your scholarship grant.
