The undersigned public attorney on May 18, 2016 requested your good office that
the undersigned public attorney be given the authority to travel for his Neuro-
Psychiatric Evaluation in Cagayan de Oro City, the undersigned public attorney
humbly amends its previous request that he be granted an authority to travel for his
Nero-Psychiatric Evaluation at Southern Philippine Medical Center in Davao City on
May 30 – 31 and to get his Ombudsman and NBI Clearance on June 1, 2016, all for
the renewal of his appointment. This amendment is for the purpose of easy travel
and to prevent further inconvenience and expense on the part of the undersigned
public attorneys since the Neuro-Psychiatric Evaluation, Ombudsman and NBI
Clearance are all located in the same area.
