AFFIDAVIT

I, POPIT TAMAYO QUISAGAN, of legal age, single, Filipino and a

resident of Brgy. Rizal, Socorro, Surigao del Norte, Philippines, having been duly
sworn in accordance with law, respectfully aver that:

1. That in my authenticated Certificate of Live Birth issued by the National

Statistic Office and Register of Births Record in the Office of the Civil
Registrar of the Municipality of Socorro, Philippines, it is stated that I was
born on 11 May 1995 at Brgy. Del Pilar, Socorro, Surigao del Norte
Philippines;

2. That I have used 11 May 1995 as my birth date in all pertinent school,
organization and voter’s registration records;

3. That when I filled up my application form for the 20 March 2016 Licensure
Examination for Teachers (LET) before the Philippine Regulation
Commission, I had correctly written the date of my birth as 11 May 1995;

4. That when I passed the Licensure Examination for Teachers, I applied for my
professional identification card last 30 June 2016 and I had inadvertently and
erroneously written the date of birth of my mother 31 August 1962 instead
of my birth date which 11 May 1995;

5. That filing Petition for Correction of Date of Birth from 31 August 1962 to
11 May 1995 was made only after lapse of time primarily because of distance
and financial constraint given that the Municipality of Socorro is located in
an island in Surigao del Norte, so that from Socorro I have to travel about
one hour to reach Brgy. Hayanggabon, Claver, Surigao del Norte, where the
van/bus going for a 3-4 hour ride to Butuan City is available. Regrettably, the
scheduled trips after the series of follow-ups I have done from March to May
2017 for such petition, were cancelled - some due to bad weather/typhoon
and some due to financial predicaments since I have no stable income yet;

6. That I am executing this Affidavit to attest to the truth of the foregoing facts
and to explain the discrepancy in my Certificates of Live Birth, Voter’s
Identification card and Professional Identification card as regards to my date
of birth;

7. Further, I am declaring that from this date hereon I would be using my true
and correct date of birth of 11 May 1995 in all my papers, records and other
documents.
