1. When you go to the Assessment Area, you MUST have with you:
a) This Test Permit in 2 copies
b) 1 pc. 2x2 size photo (with name tag)
c) Senior High School ID currently issued by your School Official (For senior high school graduating students) or any
valid ID
d) Photocopy of your Report Card with signature of your adviser
e) Photocopy of Certificate of Income of Parents or any of the following:
 Certificate of Indigency from Municipal/City Social Welfare and Development Office
 BIR Tax-exemption Certificate
 ITR or BIR Form 2316/1701
 OEC or Contract for OFW Parents
f) 1 Long White Folder
g) 1 or 2 pencil(s)
The Student Admission Committee will not allow you to take the test if you fail to bring any of the above requirements
2. You must REPORT to the Assessment Area at least one (1) hour before your scheduled time.

3. LATE COMER Examinee IS NOT ALLOWED TO TAKE THE EXAM ONCE IT BEGINS.

4. The following are PROHIBITED inside the examination rooms/premises:

a) Books, notes and other printed materials containing coded data/information/formula;
b) Programmable calculators;
c) Cellular phones, ear plugs, transmitters, portable computers, smart watches, Bluetooth and other electronic gadgets/devices
which may be used for communication purposes;
d) Bags of any kind.

5. You will not allowed to leave the examination room, borrow anything or communicate with other examinees on any
matter during examination.

I agree to the Terms of Use, have read and understood the Privacy Policy, and confirm that the information I have provided to the University are true
and correct. The submission of my SLSU Entrance Examination application form constitutes my consent to the collection and use by the Southern
Luzon State University (SLSU) of the information I have provided as well as the transfer for processing and storage at SLSU.

Furthermore, I agree and understand that I am legally responsible for the information I entered in the Online SLSU Entrance Examination Application,
and if I violates its Terms of Service, my information shall be removed and shall lead to the termination, without prior notice of my application and
everything associated with it.

I also understand that my admission in the University shall be based on passing the SLSU cut-off score, the interview, and meeting the University
requirements on a FIRST-COME-FIRST-SERVED basis, subject to the availability of slots.
