W elcome to our Summer issue of Healthy Dental Solutions. It is hard to believe that we are already
halfway through the year 2010. Where did the first decade of the 21st century go? It seems I was just
telling some of you that I now have a granddaughter. By the time you read this, she will be 18 months old.
She’s already walking and the bets are in as to whether her first words will be Mommy, Daddy, or
Mickey Mouse.

healthy medical This year has started off with some wild contradictions: some say healthcare reform is good while others
question why some countries have adapted our kind of medicine and we’re suddenly looking to change. We keep hearing about
global warming and yet early in February, 49 states had some trace of snow on the ground. The country finally got enough H1N1

vaccine just in time to see only small pockets of the flu remaining within the U.S.

So if this is how the year started, it’s hard to predict how this year and the new decade will be for us. Although I try to think
strategically on how to lead the Special Markets division into the future, I think for this year I will just take it quarter by quarter
and hope for some stability to re-enter our lives.

One of the things that has carried over to the new decade is the pain and suffering of people around us. This year we started off
with the devastation in Haiti. We have also seen more people than ever drop below the poverty line in our own country. Those of
us in healthcare must remember that we have an obligation to try to help those in need. I’m not only talking financially but more so
using the talents that we have been blessed with to help our fellow man.

I am fortunate to work for a company that continues to give back to the community and support people in a time of crisis.
Henry Schein pledged over one million dollars in supplies and aid to the people in Haiti. This number grew due to the matching
contributions of our Team Schein Members. We continue to donate products to the nation’s Community Health Centers to help
assist in the nation’s safety net for the underserved and underprivileged on a regular basis.

I tell you this as I think it is important that you know that the company from whom you buy your supplies is one with whom you

“Nu-Mem” Alveolar Socket Preservation Technique

Step 1.
Following atraumatic extraction, the socket is examined for residual debris and to check that the amount of
bleeding is appropriate. The deproteinized bovine bone grafting material is then thoroughly packed into the socket
up to the level of the crestal bone to ensure maximum bone regeneration. If an immediate implant is indicated, it
should now be inserted. In this case, there will be excess material that will be extruded as the implant is inserted

After the site has been filled to the crestal bone, the barrier material is placed over the graft. The barrier consists
of a resorbable collagen membrane. The collagen membranes are available as resorbable collagen plugs
or tapes.
If the plug is selected, it can be cut in half and the halves placed into the site one piece on top of the other,
as required. Ideally, following extraction the papillae should be undisturbed prior to filling of the socket with
bone graft material.
Alternatively, the collagen tape can be used and cut into strips before being placed in the socket(s). The
shape of the strip of tape makes it particularly useful in multiple adjacent sockets. The purpose of the barrier
membrane is to help hold the grafting material in position and to prevent contamination of the site. It has
been found that new bone will develop apical to any debris present.

Step 3.
Following placement of the barrier, the membrane or plug can be secured using either mattress or criss-cross
sutures, or both. A long-lasting resorbable suture material such as a polygalactin 910 (Vicryl®, Ethicon®; Dexon®,
Sherwood Davis and Geck), or a chromic-type suture, will ensure support of the membrane during the initial
healing period.

Step 4.
The final part of the procedure is the placement of a provisional temporary replacement over the treated
extraction site. Placing a provisional that has an ovate pontic shape to simulate the contours of the extracted
tooth, encourages soft tissue contouring that will leave a natural appearance when all treatment has been
completed. If a temporary bridge can be fashioned using two existing abutments adjacent to the extraction site,
or if a bonded temporary pontic can be used, a well-designed ovate pontic can be created. The laboratory can
use casts from recent impressions and, in discussion with the dentist, create a pontic shaped and contoured
such that it will preserve and help develop soft tissue contours during healing. A similar technique can be used
for the replacement tooth on a removable prosthesis that sits over the teeth.

Summary
As oral health improves, more people are retaining their teeth and patient expectations are increasing.
Increasingly, implants are used to replace missing teeth. Irrespective of the method used for tooth replacement,
preserving the ridge and soft tissue at extraction sites is important for both function and esthetics. Various
techniques exist for bone grafting, and for membrane use for guided tissue regeneration. A combination
technique using deproteinized bovine bone and a resorbable collagen barrier followed by use of an anatomical
temporary tooth replacement has been found to produce consistent results in preserving both the ridge and soft
tissue contours for the final restoration. As the demand for implants continues to grow, the importance of ridge
and soft tissue preservation can only increase. Use of an appropriate technique preserves alveolar ridge
anatomy, facilitates prosthetic management, optimizes function and esthetics, and enables the patient to be
treated in a shorter time with fewer surgical procedures.

Cleaning, Disinfecting and Sterilizing:


1. In operatory after use, place contaminated items in a puncture-resistant container, secure cover and transfer to
sterilization area; place all disposable sharps in sharps container at point of use (operatory), and biohazard in red
biohazard trash.
2. Process all instruments in designated sterilization area that is divided into: a) receiving, decontamination, cleaning
(debris removal); b) Preparation and packaging; c) Sterilization; d) Storage. Ideally partitions should separate the areas
to contain contaminants, or allow sufficient physical separation between areas to prevent splatter contamination.

a) Receiving, Cleaning & Decontamination:


• Deliver reusable contaminated items to designated place: in a covered container.
• Remove organic and inorganic debris by scrubbing with a surfactant, detergent and water, or use an ultrasonic cleaner
or instrument washer. To decrease risk of injury, use mechanical cleaning over manual cleaning. If you must clean
instruments by hand, scrub them using a brush with a handle, pick them up one at a time; do not reach into piles of
contaminated instruments; use tongs.
• Wear PPE to cover eyes, nose, mouth. Wear a gown and heavy-duty puncture and chemical-resistant gloves.
• After cleaning, rinse instruments with water to remove chemical residue. Use a strainer-type basket to reduce
risk of injury.
• Always handle non sterile items as contaminated.
• If unable to immediately clean instruments, place them in a puncture-resistant container with detergent,
disinfectant/detergent, or enzymatic cleaner to decrease bio-contaminant load and prevent hardening of adhesions.
b) Preparation and Packaging:
• Use a separate section for this process
• Inspect, assemble into sets or trays, package or place in container systems and wrap and label for sterilization.
Process hinged instruments open and unlocked.
• Place an internal indicator in each package, or if it cannot be seen, also use an external indicator. Follow
manufacturers’ instructions for correct placement.
• Sterilize critical and non-critical items unwrapped for immediate use only.
c) Sterilization: Only use FDA approved medial devices for sterilization.
• Use a separate section for sterilizers and related supplies with sufficient space for unloading and cooling.
• Use steam or heat sterilizers for all items that can be sterilized. Follow manufacturer instructions.
• Use dry cycle to complete process before storing items.
• Do not overload sterilizer. Place packets and cassettes on their sides next to each other to allow heat and steam to
penetrate through and around each package or container. Packages stacked on top of each other may not allow
sterilization process to items in middle of pile.
• Place chemical sterilization indicator outside and inside packets and sterilized packages. Recommend chemical
indicator that measures time, pressure and temperature. If indicator does not react correctly during sterilization, repeat
sterilization for the items.
• Use aseptic technique to transfer sterilized items from the sterilizer to storage. Achieve with use of sterile
gloves and forceps.
• Do not sterilize implantable devices unwrapped.

• Disinfect the handle and controls of the sterilizer after starting cycle to prevent post-cycle contamination when opening
and removing sterile items from the sterilizer.
• Critical and Non-Critical items must be dried by the sterilizer if they are stored before use. Wet packaging is porous
and allows contamination.
• Heat sensitive items can be completely sterilized through complete immersion in an FDA approved steriliant/high-level
disinfectant. Make sure you use a product designated for this purpose and follow manufacturers’ instructions. Process
can take up to 12 hours. Keep cold-sterilie container lidded to prevent vapor irritation.
• When cold sterilization is complete, rinse items with sterile water to remove chemical residue, dry with sterile towels
using sterile gloves, and transport to point of use in aseptic manner.

• Do not use cold sterile solution as a surface disinfectant.

• Match product to the purpose: Use all chemicals, cleaners, disinfectants for the purpose designated and follow
manufacturers’ use instructions. Keep a copy of the MSDS sheet for safety instructions and compliance.
• Do not use bead sterilizers. The FDA has determined they are ineffective.
• Use a Biological Indicator (BI) at least weekly or as mandated by state law, to check effectiveness of sterilization
‘kill’ of pathogens.
• If your biological indicator (BI) activates during sterilization, run a second cycle with the BI. Common reasons for BI
failure include: overloading, failure to provide adequate package separation, loading, spore testing. Often correcting the
process will resolve the reading on the second cycle. Keep items in quarantine until you have determined if the
sterilizer has malfunctioned.
• Record results of BI monitoring and corrective actions as part of the overall infection control program. Many sterilizers
have record print ability. Maintain records in compliance with Federal, state and local laws and regulations.
• When process tests show sterilizer is faulty, remove it from service, repair or replace as appropriate.
• The CDC has established protocols for positive BI tests.

d) Storage of Sterile Items:


• Keep sterile and disposable items in dry, enclosed storage.
• If packages tear, open or get wet before use, re-package and completely re-process contents.

Disinfection of Treatment Areas:


• Identify all contaminated (clinical contact) surfaces: light handles, switches, pens, X-ray cones, arms and exposure
buttons, chairside keyboards, knobs, tubing, counters, drawer handles, faucets, lead aprons, etc.
• Clean all such surfaces with detergent or surfactant and rinse to remove debris.
• Wet disinfect surfaces with an EPA-registered hospital disinfectant with HIV, HBV (low level disinfectant) claim or a
tuberculocidal (intermediate disinfectant) claim. Areas that were visibly contaminated with blood or OPIM need the
latter. Allow wet surfaces to evaporate to dry.
• Check equipment manufacturer cleaning recommendations to ensure disinfectant compatibility to reduce damage to
upholstery and other surface materials, and prevent voiding of warranties.
• Use protective, disposable barriers on clinical surfaces whenever feasible and change between patients.
• Non-clinical (house-keeping) contact areas such as walls and floors should be washed regularly with an appropriate
cleaner or an EPA registered hospital disinfectant/detergent.
• Wear appropriate PPE when cleaning and disinfecting environmental surfaces

he stood holding a bib chain in her hand, gently swirling it as I prepared lunch. We often brought the clip into the
kitchen to save our street clothes from the ravages of baked ziti and the turbulence of tamale pie. I watched the
chain go back and forth and realized that something was coming from it. On closer examination, we discovered that

debris was falling from the balls that made up that chain. Bib clips and other items with hidden reservoirs can be a
source of cross-contamination when they come in contact with moisture or dislodged debris and contaminate the patient
or environment.

The Centers for Disease Control and Prevention defines cross-contamination as the act of spreading bacteria and viruses
from one surface to another.
Since blood-borne viruses can live on objects and surfaces for up to a week, germs could be spread when surfaces are not
disinfected the right way or if equipment is not cleaned and sterilized between patients.

Each day that I put that clip in place, I felt guilty and decided it was time to find out exactly what risks I was taking and
developed a simple solution to my observation. I decided to agar plate a variety of clips and see exactly what I was sharing
with my patients. I gathered 12 clips from a variety of offices and placed them in sterile pouches, plated them out and did a
count of the results after 48 hours of incubation. Each office reported they had disinfected the item using a hospital grade
disinfectant but had never sterilized the item or put it in the ultrasonic bath.

CFU is a measurement of colony forming units of viable bacteria or fungal numbers. The method I used was to agitate the
clips in sterile saline and plate out on agar (see related chart). It was incubated for 48 hours and a count was done.
The count is given in CFU/mL (colony forming units per milliliter). The Environmental Protection Agency has established a
limit of 500 cfu/mL for drinking water.

What I discovered:
• Bib clips and other items can be a source of cross-contamination. In their crevices, they can harbor pathogens.
• When the hollow bib clip was placed in solution, it released debris out of the tubing and had some of the highest CFU/mL
counts. In the moist dental environment we work in, cross-contamination could occur in the same manner.
• The larger the inaccessible area, or “hidden” surface area the higher the CFU/mL count. The clip that had a plastic mouth
with many crevices and indentations and the chain that had flaking paint and an inaccessible plastic ring had the
highest counts.
• The solid rubber bib clip with limited indentations and crevices had the lowest number of CFU/mL.
• The autoclaved and the disposable bib holders did not create enough CFU/mL to be considered a source of viable
pathogens.

Solutions
• Use disposable bib holders. They are simple to use, cost effective, and limit cross-contamination.
• Use bib holders with no chains, limited embossing, and no crevices. Do not buy clips that cannot be sterilized (most can).
Put one in each setup, run through the ultrasonic bath to remove debris, and then sterilize.
• Do not bring any dental equipment into the area set aside for food in the dental setting. The chance of cross-contamination
is not worth the risk.
Bib clips and other items with hidden reservoirs can be a source of cross-contamination. Every healthcare professional has
the responsibility of protecting patients and themselves from the risk of cross-contamination.
