3 - Equipment malfunction (If any) - Expose of radiation to worker and - Damage to body sel and blood sel - Radiographer shall notify RPS
public - Radiographer measure radiation level around barricaded area to ensure
dose rate less then 0.25mR/hr
- RPS will initiate Emergency Response Plan

4 - Radiation work start operation - Expose of radiation to workers and - Damage to body or blood sel - Make sure the dose receive is below 0.75mR/hr for worker & 0.25mR/hr for
public public (monitor using radiation survey meter)
- Survey all barricade area is below 0.25mR/hr
- Used lead sheet as a shielding if radiation is to high
- Stay behind any structural to reduce radiation exposure
- Used survey meter, warning light & alarm to monitor radiation exposure

9 - Position the dog house to radiation - Pinch point - Injuries to hand or finger - Always be alert of what you are doing and stay focus at all time
work area (if dog house is used) - Use the over head chain block and tag line to move the dog house
- Expose to radiation - Damage to body and blood sel - Position the dog house first before removing the tracker pill from pipe
- Be careful while position the dog house to the pipe this is to make sure it don't
hit and drop the tracker pill which will cause the crawler to x-ray

10 - Damage tracker pill or Equipment - Expose of radiation to worker and - Damage to sel tissue and blood sel - Radiographer shall notify RPS
malfunction (if any) public - Radiographer measure radiation level around barricaded area to ensure
dose rate less then 0.25mR/hr
- RPS will initiate Emergency Response Plan

11 - Radiation work complete or pipeline - Heavy load - Back injuries - Proper way of lifting & handling of heavy load. Ask for help or use crane
finish laying - Pinch point - Injuries to hand or finger - Always be alert of what you are doing and stay focus at all time
- Trip and fall - Injuries to personnel - Walk carefully & don't run. Removed any tripping hazards
