and signature (e.g. Company ID, Driver’s License, Passport, Professional Regulation Commission (PRC) ID, SSS ID, BIR ID,
Voter’s ID, etc.) via email at cardactivation@eastwestbanker.com or fax at (02) 325-2377. Should you have any concern, you
