Gentlemen,

This will authorize you to debit my/our Savings/Current Account No.

__________________________________ with your __________________________________ Branch
(hereinafter referred to as the “Account”) for the amounts necessary to cover the amortizations and or
obligations arising from any and all promissory note/s and/or any real estate and/or chattel mortgage
contract/s which I/we and/or SPS. JESUS BARADAS VALDEZ & ANN-GELYN ADREMESIN VALDEZ
have executed/will execute in your favor under such [home/small business/auto/etc.] loan and/or other
loan facilities obtained by me/us and/or SPS. JESUS BARADAS VALDEZ & ANN-GELYN
ADREMESIN VALDEZ from you, on the date that said amortizations and/or obligations fall due. Such
amounts shall include, but shall not be limited to, premium payments on insurances, interests penalties,
charges and such other amount/s as may be due to you under the said contract/s/loan facility/ies, and
this authority shall likewise include escalations, accelerations or adjustments which you may impose
pursuant to the terms and conditions of the aforesaid notes. mortgage/s, contracts or facility/ies.

This will further authorize you to apply the debited amount to any of my/our and/or SPS. JESUS
BARADAS VALDEZ & ANN-GELYN ADREMESIN VALDEZ loan accounts or any other obligations
with you as you may deem proper.

I/we undertake to deposit sufficient funds to the Account to cover the foregoing monthly
amortizations and/or obligations and agree not to close the Account until all my/our and/or SPS. JESUS
BARADAS VALDEZ & ANN-GELYN ADREMESIN VALDEZ obligations to you have been fully paid.

I/we hereby authorize you or any of your duly authorized representatives to conduct any
necessary inquiry and/or examination of the Account for the purpose of effecting the implementation of
this automatic debit agreement.

I/we shall indemnify and save you and/or your authorized representatives free and harmless
from any and all liability arising out of any violation of the Law on the Secrecy of Bank Deposits, the
provisions of the General Banking Law, and other related laws and/or government regulations on the
subject of disclosure of bank/transactions.

I/we acknowledge that the authority granted herein shall not in any manner prejudice any of the
rights and privileges granted to you under any promissory note, the real estate mortgage and/or chattel
mortgage and other related documents executed in your favor in relation to the foregoing loan/mortgage
transaction/facilty/ies, or your right to terminate this agreement at your option in case of my/our and/or
SPS. JESUS BARADAS VALDEZ & ANN-GELYN ADREMESIN VALDEZ default or violation of the
terms and conditions thereof, or for any reason whatsoever, in any manner at your absolute option and
discretion.
