PETITION FOR NOTARIAL COMMISSION

Petitioner, JUAN DE LA CRUZ, respectfully submits his Petition for

the Issuance of Commission as Notary Public for the Province of Misamis
Oriental and the City of Cagayan de Oro. In support of this Petition, he
respectfully states the following:

1. That petitioner is of legal age, having been born on 20

February 1986, single, Filipino citizen, a resident of the Republic of the
Philippines since birth and presently residing at ___________________
(ADDRESS/RESIDENCE), Cagayan de Oro City;

2. That petitioner has satisfactorily completed the required course

in law at Liceo de Cagayan University in the year 2015 and eventually
passed the 2016 Bar Examinations;

3. That petitioner is a bona fide member of the Integrated Bar of

the Philippines, particularly the Misamis Oriental Chapter, with Roll of
Attorneys No. ___________________ and IBP No.
___________________. As proof of said facts, a copy of the
Certificate of Membership and the IBP Receipt dated
___________________ are attached and marked herewith as Annex “A”
and Annex “B” respectively;

4. That petitioner is a member of good standing of the Philippine

Bar as shown by the following certification, to wit:

a) Certification of No Pending Administrative Case

issued by the Office of the Bar Confidant, a copy of
which is attached and marked herewith as Annex “C”;

b) Certificate of Good Standing issued by the IBP –

Misamis Oriental Chapter, a copy of which is attached
and marked herewith as Annexes“D-1”and “D-2”;

5. That petitioner has also paid the Professional Tax Receipt

due him as evidenced by PTR No. ___________________, dated
___________________, a copy of which is also attached and marked
herewith as Annex “E”;

6. That petitioner has not been charged with, much less,

convicted of any offense involving moral turpitude. As proof that no criminal
case has been filed in the Philippine courts against him, a copy of his
National Bureau of Investigation Clearance with NBI ID No.
___________________ valid until ___________________ is also
attached and marked herewith as Annex “F”;

7. That petitioner is presently holding office at

___________________ (ADDRESS/RESIDENCE), Cagayan de Oro City;

8. That in view of the foregoing, petitioner has all the

qualifications and none of the disqualifications for the Office of the Notary
Public;

9. That in addition to the above attached and marked documents,

petitioner has attached hereto, in a separate sheet of paper, specimen of
his official signatures, passport size photographs, and the sample
impressions of his dry seal marked as Annexes “G”, “H” and “I”,
respectively and made an integral part hereof;

10. That Having recently passed the 2016 Bar Examinations,

petitioner is exempted from the Mandatory Continuing Legal Education as
mandated by the Supreme Court;

12. That petitioner has no commission as notary public in any other

jurisdiction;

13. That petitioner has not been issued an Appointment as Notary

Public by this Honorable Court ever since and that this is the first time that

as Notary Public for and in the City of Cagayan de Oro, and the Cities and
Municipalities of the Province of Misamis Oriental.
