HRD LDNA FORM 2: Learning and Development Needs Assessment Tool for
Teachers on
Financial Status

The Department of Education (DepEd) in April 2018 has formalized


partnership with the Government Service Insurance Information System (GSIS) with
the aim of promoting the welfare and improving financial capability of teaching and
non-teaching personnel. This paved way to the GSIS Financial Assistance Loan.
In view of the Division of Biñan City’s efforts to take part in the mission of
helping personnel to be free from the burden of loans and over-borrowing, the
Human Resource Development (HRD) Section of the School Governance and
Operations Division would like to know about the following details included herein.
The inputs to be obtained will be the basis for HRD-related intervention and
employees’ welfare programs.
We request you to answer as honestly as possible so that we can get
accurate data. Your school headand the Coordinator for Learning Advancement and
Development in School (Co-LEAD) will explain to you the directions for
accomplishing the form. Rest assured that all details will be treated with full
confidentiality and respect.

Directions:
1. The Coordinator for Learning Advancement and Development in School
(Co-LEAD) or any other personnel assigned by the school head shall take
the lead in accomplishing this survey form.
2. The Co-LEAD prepares a list of all the school personnel in excel format,
bearing the school header. The format should have three columns for the
Names (last names first), Code (school ID-001 for the first on the list, and
so on), and position.
3. All teachers should accomplish everything in the form except the code
which the Co-LEAD will provide prior to distribution. Names of banks and
other lending agencies need to be specified. If the loan is borrowed from a
person, indicate ‘personal’. Current balance need not be based on the
statement of account as this may be computed by multiplying the monthly
deduction to the number of remaining months to pay.
4. Other sources of income may include business, tutorial and other income-
generating sidelines.
5. The Co-LEAD shall consolidate the report in four columns in excel format:
number, code, position, and total loaned amount. The hard copy of
individual forms need to be attached and submitted to the SGOD Office on
or before May 4, 2018. Soft copy needs to be sent to
depedbinan.hrd@gmail.com
6. The school head ensures that all employees have complied, signs the
summary and submits the hard copy at the Division Office.
