We, RIZA C. RODRIGUEZ and LUCILLE D. SABANDAL, Both of legal age,

Filipino, married and single, respectively, and residents of Brgy. San Roque,
Macrohon, Southern Leyte, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and state;

1. That we personally know the personal identity of WELYN PACAONCIS of

San Roque, Macrohon, Southern Leyte;

2. That she was born on January 18, 1978 at Nato, Esperanza, Agusan del
Sur;

3. That however there are discrepancies in her records as the were recorded
as follows:

with Sex as FEMALE and Date of Birth as January 18, 1978 per
information in her all of her official records, refer only to one and the same
person;

5. That we know these facts because we live in the same barangay and close
friends of their family;

6. That we are not related to them within the fourth civil degree, either by
affinity or consanguinity;

7. That we are executing this affidavit to attest to the veracity of the

foregoing facts and for any legal purpose this may serve thereby.
