Date Activities

 8.30-8.50am: Trainee was greeted by cosmo scientex personnel (Mr.

Bryan) and working uniform was distributed and briefly explained about
the importance of attire such as long sleeve uniform, boots, safety
helmet, and eyeglasses to maintain a safe working environment.
 8.50-9.00am: All employees were joining daily exercise routine. This
10 minutes exercise is a Japanese discipline and was implemented by
the company to encourage employees to maintain a good health and
physicality to do work.
 9.00-9.15am: Trainee attended a daily morning safety call to gather
11-06-2018 employees and updated all the work done, problems or any
(Monday) announcement. Trainee also introduced during this session.
 9.15-9.45am: Trainee was given a safety and health book for newcomers
so that trainee is acknowledge on safety precautions during work or
upon inspection.
 10.00-1.00pm: Trainee was having a meeting and briefing session with
head of department (maintenance & utility), Mr. Shahron. This session
aimed to brief trainee about company profile, main activity in the plant
and job scopes under the department.
 1.00-2.00pm: Rest.
 2.00-6.00pm: This session introduces trainee to the production plant of
cosmo scientex. This is the opportunity for trainee to be exposed to the
real industry in terms of engineering tools, reactor, working principal,
utility and what do and don’ts while in the production plant. Trainee was
exposed mainly on how the maintenance department play a role inside
the plant. This session also gave the opportunity for trainee to be
introduced to cosmo scientex staff.

 8.50-9.00am: Daily exercise routine was participated by trainee

 9.00-9.15am: All employees required to attend safety morning call
 9.30-11.30: Meeting with Maintenance Engineer in the maintenance &
utility department, Mr. Faiz was held in cosmo scientex company office
to explained in detail about maintenance engineer job scopes and routine
need to be done during maintenance.
 11.30-1.00pm: Another plant visit was held specifically exposed trainee
to the main maintenance working parts such as the utility (cooling tower
12-06-2018 etc.), melting bath (where solid form of raw material was melted for
(Tuesday) production), nitrogen liquid tank, storage area, hopper and scheduled
waste management.
 1.00-2.00pm: Rest.
 2.00-6.00pm: Maintenance job scope was explained further: receive
report form regarding mechanical abnormalities (reactor, pump etc.)
from production team and record and check status if already done or not.
safety presentation required maintenance department to go inside plant
 to observe any hazard potentially occur during the operation and need
to determine what is the prevention. Maintenance department will be
having a monthly inspection to check all unit operation involved in the
process is in good condition. any abnormalities are recorded in the
checklist form. During booster pump check, the body temperature
detected was 80°C but the inlet and outlet both in 42°C and 43°C
respectively. when the valve at the outlet is open no water dispersed,
meaning the booster pump is not working well and no water is fed into
the pump. Agitator also have been inspected in terms of temperature,
sound, vibration to record any abnormalities or defect.

 8.50-9.00am: Daily exercise routine was participated by trainee.

 9.00-9.15am: All employees required to attend safety morning call.
 9.30-1.00pm: This period was used for trainee to study the company
profile and review the work given by supervisor. “Safety Presentation”
13-06-2018 is a monthly presentation by maintenance & utility department focusing
(Wednesday) on hazard and countermeasure in the work place. Trainee was given a
task to complete “Kiken Yochi Training” where in a situation of
maintenance/construction work is conducted, the hazard meets the work
was observed and the countermeasure of the hazard was determined.
 1.00-2.00pm: Rest.
 2.00-6.00pm: “Kiken Yochi Training” resumed and daily activities
report was recorded.

 8.50-9.00am: Daily exercise routine was participated by trainee.

 9.00-9.15am: All employees required to attend safety morning call.
 9.15-1.00pm: Weekly report was recorded and organized. Trainee was
14-06-2018 given a task to perform “Patrol Comment” where inspection of previous
(Thursday) problems aroused have been done or still in the previous condition.
 1.00-2.00pm: Rest.
 2.00-6.00pm: “Patrol Comment” was conducted with the help of cosmo
scientex staff to locate the problems given and power point presentation
was prepared and sent to maintenance & utility staff.
