A. PUPIL’S DEVELOPMENT

1. Administered 1st to 4th periodical test with very satisfactory results.

2. Carylle G. Bandayrel garnered 1st Place in drawing competition during Buwan ng Nutrisyon.
3. Another certificate garnered by Mackrist Keigo M. Fontanilla won Pinakamagandang
Kasuotan (Ginoo) - and Pinakamagandang Kasuotan (Binibini) - Shayne O. Clarito and at
the same garnered Bb. Buwan ng Wika during the Celebration of Buwan ng Wika last August
17, 2017 at Canarem Elem. School, Natividad, Pangasinan.
4. Jay Raven L. Jonelas won as Ginoong Buwan ng Wika during the celebration last August 17,
2017.
5. Girl Scouts won 3rd Place in Calesthenics during the Girl Scouts Week Celebration at Umingan,
Pangasinan last September 23, 2017.
6. October 13, 2017, Mackrist Keigo M. Fontanilla, won the 2nd place in Environmental Quiz in
Science 3 and also Carylle G. Bandayrel for 4th Place during the District YES-O Camp &
Leaders Summit at San Miguel Elem..School, Natividad, Pangasinan.
7. During the District YES - O Camp & Leaders Summit, another certificate garnered by Mackrist
Keigo M. Fontanilla won the title Mr. Enviro-Hero, Little Mr. Environmental - Fire, Best in
Talent, and Best in School Uniform.
8. Another certificate garnered by Carylle G. Bandayrel won the title Little Ms. Environmental -
Earth, and Best in Talent.
9. During the United Nations, Mackrist Keigo M. Fontanilla won the title Best in Costume (the
Astronaut Costume) last October, 2017.
10. Remedial Reading was given to slow readers and remedial teaching to the slow learners.
11. Conducted Saturday Math Class for six (6) Saturdays

B. TRAININGS/ SEMINARS

1. Attended and participated actively during the Division Program Implementation Review held
on April 27, 2017 , at the 3rd Floor Amethyst Hall, Pentagon Building, Urdaneta City,
Pangasinan.
2. Participated actively during the 3-day District Learning Action Cell Session Focused on Oplan
Balik Eskwela, Daily Lesson Log, Individual Performance Commitment and Review Form
and Code of Ethics at Natividad Central School, Natividad, Pangasinan on June 2-3 & 10,
2017
 3. Attended the Orientation-Workshop on the Policy and Guidelines on Healthy Food and
Beverage Choices in Schools and in DepEd Offices, School Based Feeding Program School
Year 2017 -2018 and Training on Food Safety , held at Angela Valdez Ramos National High
School, Asingan, Pangasinan last July 25, 2017.
4. Have attended the Division Conference for Canteen Teachers held at Eastern Pangasinan
Agricultural College (EPAC) , Sta. Maria, Pangasinan on September 24, 2017.
5. Attended and participated actively during the Three-Day District Seminars-Workshop on
Effective Strategies on Improving Reading Abilities of Non-Readers and Slow Readers and
Effective Strategies in Teaching Language Arts for Elementary School Teachers, held at
Natividad Central School , Natividad, Pangasinan on October 28-30, 2017.
6. Attended the Division Program Implementation Review (PIR) on School Based Feeding
Program (SBFP) S.Y. 2017 - 2018 at Villasis Gymnasium, Villasis, Pangasinan on 1st day of
December, 2017.

C. AWARDS

1. Received a Certificate of Recognition for having garnered as facilitator during the 2017
District Youth for Environment in Schools Organization (YES- O) Camp and Leaders’
Summit, held at San Miguel Elem. School on October 13-15, 2017.
2. Winning Coach in Pinakamagandang Magandang Kasuotan (Ginoo and Binibini) during the
celebration of Buwan ng Wika held at Canarem Elem. School on August 17, 2017.
3. Winning coach for Ginoo Buwan ng Wika and Binibining Buwan ng Wika last August 17,
2017.
4. SILVER- SERVICE AWARDEE for continuous service and performance as Troop Leader for six
(6) consecutive years on September 23, 2017 during the Triennium 2015 - 2018 at Umingan
Public Auditorium, Umingan, Pangasinan.
5. Another winning coach in Calesthenics for garnered 3rd Place during the Girl Scouts Week
celebration held at Umingan Gymnasium, Umingan, Pangasinan on September 23, 2018.
6 Another winning coach 2nd Placer and 4th Placer in Environmental Quiz ( Grade 3 )during the

October 13, 2017 ,

& Leaders Summit at San Miguel Elem. School, San Miguel, Natividad, Pangasinan.
8. Another winning coach for Little Ms. Environmental - Earth ,and Best in talent by Carylle G.
Bandayrel during the YES - O Camp and Leaders Summit celebration last October 15,
2017.
9. One of the trainers in Saturday Math Class for S. Y. 2017 - 2018.
10. Award of Recognition for participation during the 43rd Eastern Pangasinan Council Jamboree
on February 10, 2018 at Brgy. Legasp, Tayug, Pangasinan for 3rd Place in

D. CLASSROOM DEVELOPMENT PROJECTS

 1. Put up the lavatory and bowl procured from the PTA Class SY. 2017 - 2018 for the Restroom
of Grade 3 , at Canarem Elementary School, Natividad, Pangasinan.

1. Conducted School- Based Feeding Program at Canarem Elementary School for six months
(August to December, 2017)
2. Participated actively during the Town Fiesta in Tayug , Pangasinan last March 15 - 19, 2018.

through Multimedia on October 28, 2017 at Tayug South Central E/S, Tayug, Pangasinan.
 Have attended the Division Program Implementation Review (PIR) on School - Based Feeding
Program S.Y. 2017 - 2018 at Villasis, Gymnasium, Villasis, Pangasinan on December 01, 2017.

 Received an Outstanding Teacher Award for Outstanding Contribution and Dedication to the
Academic Achievement on October 3, 2017 at Tayug Central School, Tayug, Pangasinan.
