Nurse 1 : “Good morning Mis, can I help you ?”

Patient : “Good morning, I want to check up, where should I go ?“
Nurse 1 : “oke..Let me introduce my self, I am Kuntari Arum Sari, what’s your
name ? “
Patient : “My name is Dian Widyastuti“
Nurse 1 : “what do you feel mis ?“
Patient : ” Already these past few days, my lower part right abdomen is very
painful “
Nurse 1 : “Approximately already how many days you felt this pain?”
Patient : “Already this past week”
Nurse 1 : “Can you remember the cause of the pain?”.
Patient : “I do not know for sure the cause, but I like to eat chili “
Nurse 1 : “How severe and often painful are you feeling? “
Patient : “Every time and it feels like being sliced “

consultation with doctor about your patient which name is Mrs Dian.
This is the result of her CT-Scan and her medical Record.

Doctor : “Okay briefly I check first the x-rays. And data patient?”
Nurse 2 : “Dian Widyastuti, 35 years old, check in to this hospital on April 15th
2018, medical diagnosis not known yet, the complain is currently feels
 pain in abdomen and had known the patient like to consume spicy
food”.
Doctor : “What is the scale of pain felt by patient?”.
Nurse 2 : “The pain scale is five doc”.
Doctor : “Then what else is the data obtained?”.
Nurse 2 : “there is pain on the bottom right the abdomen and there is a lessions
on it. Patient even said that pain was increases when she used for
walk”.
Doctor : “From the results of x-rays, patients have appendicitis caused by chili
seeds that can’t be destroyed by the digestive system and cause
buildup”.
Nurse 2 : “I also concluded like that during the assesment”.
Doctor : “Immediately notify the patient's family for surgery. Because if not
immediately operated the appendix will explode and will cause more
