No fatality observed in this accident but the driver of the third party trailer was injured. Probable cause of accident
1. After exploring the accident spot and taking statements of eye witnesses, It was observed that both the
driver were driving the vehicle with gradual speed but they were not maintaining the safe distance between
both vehicles, which caused the damage of front vehicle also.
5.Route Map 2. Rear visibility of the carrier was below the standard and was not as per CMVR (Poor quality of reflective
tape and triangle).
3. Speed of the third party vehicle was above the limit and driver was not able to control the vehicle after
minor side collision with JCB and then collided with IYM truck.

Recommendation
1. Drivers should be encouraged by the transporter to attend DDT whenever it is conducted at IYM. Transporters should make sure that all drivers involved in IYM operations should be DDT trained.
2. Transporter should monitor the driver whether he is not driving at night which is major cause of fatigue.
3. This is requested to IYM that all the vehicle must have the reflecting tape and triangles and it should be monitored each time before loading with NO GO clause since it plays the important role during dark
4.Follow all DDT methods on road i.e. Safe following distance between two running vehicles (3 second rules)
5. This is also requested to IYM that co-driver /helper must be mandatory along with driver to avoid other mishappening in future.

maximum of damaged CBU)- No of the CBUs could not be identified
