RE: APPLICATION FOR RENEWAL OF

NOTARIAL COMMISSION

Dear Madam:

Greetings!

I am a citizen of the Philippines, born on February 23, 1991, single and

a resident of Block 7, Lot 7, Purok 17, Casisang, Malaybalay City,
Bukidnon. My office number is (088) 813-3349 and my mobile number
is (+63) 9177038190.

I received my undergraduate diploma in AB Political Science from

Central Mindanao University in the year 2011 and completed my LLB
at Xavier University – Ateneo de Cagayan on the year 2016.

I passed the 2016 Bar Examination given on November, 2016. I was

admitted to the practice of law in the Philippines on May 23, 2017 and
signed the Roll of Attorneys under Roll of Attorneys No. 67495. A
machine copy of my Certificate of Membership in the Philippine Bar is
hereto attached as Annex “A”.

I was previously granted a Notarial Commission which will expire on

December 31, 2018, under the Notarial Commission No. 44-17.
I wish to renew my commission as a Notary Public for and in the
Province of Bukidnon.

I possess all the qualifications and none of the disqualifications for the
said office.
I have never been convicted of any crime involving moral turpitude.

I am a member of the Philippine Bar in good standing and clearances

from the Office of the Bar Confidant of the Supreme Court and the
Integrated Bar of the Philippines, machine copies of the said clearances
are hereto attached as Annex “B” and “C”, respectively.

I also paid my Professional Tax Receipt for the year 2018 with PTR
Number 6933714 paid on January 15, 2018. I paid my IBP dues in the
year 2018 on January 10, 2018 bearing an Official Receipt No.23743.

I have complied with the Mandatory Continuing Legal Education

(MCLE) requirement for the sixth (6th) compliance period, with MCLE
Compliance No. VI-0011647 issued on August 18, 2018.

Finally, I have no criminal nor any other derogatory record as

evidenced by the National Bureau of Investigation (NBI) Clearance,
issued on November 06, 2018, a machine copy of which is hereto
attached as Annex “D”.

In view of the foregoing, I respectfully pray that my Notarial

Commission for and to the Province of Bukidnon be renewed for the
period of January 1, 2019 to December 31, 2020.
