Bills Payment Slip.
3. Proceed to the teller with the Bills Payment Slip and this letter for reference. If paying by check, please make
sure it is payable to BDO INSURANCE BROKERS, INC. The teller will validate the Bills Payment Slip and it
will serve as your proof of payment.
4. Fax the validated bills payment and this renewal advice (with signed confirmation below) to
(02) 866-6999. Please address the fax to: CLG MRI RENEWAL TEAM.
