Corrigendum on the CONDUCT OF MONITORING AND EVALUATION OF

SCHOOLS PHYSICAL ASPECT (CLASSROOMS AND ADMINISTRATION OFFICE)
(Division Memorandum No. 23, s. 2019)

ADVISORY
This is to inform the field on the corrigendum on the Conduct Monitoring & Evaluation
of Schools in Physical Aspect (Classroom & Administrative Office (Division Memo No. 23, s.
2019) .

1. Rating Scale for the School Head

4.500-5.000 - Outstanding
3.500-4.499 - Very Satisfactory
2.500-3.499 - Satisfactory
1.500-2.499 - Poor

2. Rating for the Teacher/Kinder Teacher

(94-100) - 5 - Outstanding
(88-93) – 4 - Very Satisfactory
(82-87) – 3 - Satisfactory
(75-81) – 2 - Unsatisfactory
(Below 75) – 1 - Poor

3. The school will reproduce the evaluation tools/criteria

4. For the relief teachers, the evaluator will focus only on the applicable criteria
divided by their total score.
5. The evaluators will conduct a short debriefing with the School Heads and
Teachers after the evaluation is done.

Wide and immediate dissemination of this Advisory is desired.
