owner & 7/12 Copy(for Non MIDC) 4 MIDC agreement and possession
receipt(for MIDC) 5 grampanchayat NOC copy in name of Company(for
Non MIDC) 6 Final drawing copy of factory and layout copy 7 Declaration
for the investment By Chartered accounttant (Format attached) 8 water
budget means input water, uses, applications, lossess, recycled,
Effulent(format attached) 9 Manufacturing process of each product in
detail. 10 Process flow diagram 11 SSI Regn. Copy Or IEM Regn. Copy 12
M.O.A. Copy 13 List of Raw Material With monthly or annual consumption
14 List of manufacturing Products with monthly or annul quantity 15
Details of the Genrators or boilers(Capacity,Fuel type and quantity) 16
Details of the Stack and the chimeny(Height,Portol Diameter ) 17 Details of
Hazardous/Non Hazardous waste Type of waste,Quantity and details of
disposal 18 Details of ETP/STP Process and scheme diagram 19 Details of
  Capital Investment of industry (including land, building, machinery without depreciation)
 Mode/type of Operation/Business.
 Ownership details.

Also further following docs. will be required during consent application process-

1] For Consent to establish:

o Site plan/index
o Topographical Map
o Detailed layout plant of different processes and point sources of effluent
discharge/emissions and position of stack and documents including D.G. set capacity
in KVA.
o Process flow sheet.
o Details of Water Pollution Control/Air Pollution Control devices proposed to be
provided.
o Ambient Air Quality Report (if available)
o SSI Certificate/NOC from Directorate of Industries Government of Maharashtra.
o D.G.T.D. Registration. (if applicable )
o Details of chemical reactions with mass balance.
o Consent fees in the form of D.D. drawn on favour of MPCB.
o Local body NOC.
o Under taking on Rs. 20 stamp paper or Chartered Accountant certificate about
proposed Capital Investment ( Land, building, and machineries.

2] For Consent to Operate/Renewal:

o Detailed layout plant of different processes and point sources of effluent

discharge/emissions and position of stack and documents including D.G. set capacity
in KVA.
o Process flow sheet.
o Latest analysis report of effluent, fuel gases, solid waste & hazardous wastes.
o Details of Water Pollution Control/Air Pollution Control devices provided.
o Ambient Air Quality Report (if available)
o SSI Certificate/NOC from Directorate of Industries Government of Maharashtra.
o D.G.T.D. Registration. (if applicable )
o Details of chemical reactions with mass balance.
o Consent fees in the form of D.D. drawn on favour of MPCB.
o Xerox copy of previous consent (for renewal only).
o Xerox copy of Environmental Clearance of Government of Maharashtra or
Government of India in case of 1st consent to operate in case of industries/process
requiring environmental clearance.
